Abstract: Summary Management of pulmonary embolism with haemodynamic instability Pulmonary embolism with haemodynamic instability is associated with high mortality rate despite the progress made in recent years. The article deals with pathophysiology of acute respiratory and cardiovascular failure as well as diagnostic strategy for unstable patients with pulmonary embolism. Treatment includes respiratory and haemodynamic support, anticoagulation, thrombolytic therapy as well as surgical embolectomy or percutaneous catheter-directed interventions.
Rezumat Managementul emboliei pulmonare cu instabilitate hemodinamică Embolia pulmonară cu instabilitate hemodinamică reprezintă o cauză importantă de mortalitate. În articol sunt elucidate mecanismele fiziopatologice ale insuficienței cardiace și respiratorii cât și algoritmul de diagnostic al pacienților cu embolie pulmonară și instabilitate hemodinamică. Terapia intensivă constă în asigurarea unui suport respirator și hemodinamic, administrarea anticoagulantelor și tromboliticelor, efectuarea embolectomiei chirurgicale sau a intervenţiilor percutane direcţionate pe cateter.
