Abstract: Rezumat Vasele supranumerare (aberante, accesorii) reprezintă cea mai frecventă cauză a stricturilor joncţiunii pielo-ureterale (SJPU) prin mecanism extrinsec, ele provoacă conflict vaso-ureteral cu obstrucţia fluxului urinar. în mod prospectiv au fost analizate 48 cazuri de SJPU prin mecanism extrinsec provocat de prezenţa vaselor supranumerare, tratate chirurgical în Clinica de Urologie a IMSP Spitalul Clinic Republican între anii 2008 şi 2013. Rezecţia vasului supranumerar cu sau fără ureteroliză a fost efectuată în 35,4%, pieloplastii antevasale - 20,83%, pieloplastii cu rezecţia vasului supranumerar - 27,1%, nefrectomii - 16,66%. Tratamentul chirurgical de elecţie este pieloplastia cu sau fără rezecţia vasului polar. Acest tip de intervenţie este corect din punct de vedere al impactului morfologic realizat de compresia vasului supranumerar pe segmentul pielo-ureteral.
Summary Crossing Vessels (CV) are the most frequent cause of extrinsic ureteropelvic junction obstruction (UPJO), they cause vaso-ureteral conflict and obstruction of urinary flow. We analyzed prospectively 48 cases with extrinsic UPJO, caused by the presence of crossing vessels treated in the Department of Urology, Republican Clinical Hospital between 2008 and 2013. Resection of crossing vessel with or without ureterolizis was performed in 35,4%, antevasal pyeloplasy in 20,83% pyeloplasty with resection of crossing vessel in 27,1%, nephrectomy in 16,66%. Pyeloplasty with or without resection of the crossing vessel is the surgical method of choice for patients with extrinsic UPJO. This type of surgery is corectly in terms of morphological impact of the compression made by crossing vessel onUPJ.
