AbstractScop: analiza particularităţilor şi limitărilor de administrare a ACO bolnavilor cu FA şi risc tromboembolic înalt. Metode: Cercetarea prospectivă a 81 pacienţi cu FA non-valvulară (persistentă sau cronică) şi risc tromboembolic înalt (CHADS2≥2). Rezultate: Vârsta medie a pacienţilor cercetaţi a constituit 59,8±0,84 ani, 60,5% fiind bărbaţi. În grupul de studiu prezentat, 56,8% pacienţi aveau forma cronică a FA, iar 43,2% - FA persistentă. Excluzând persoanele cu contraindicaţii pentru ACO, iniţierea terapiei cu warfarină s-a reuşit la 88,9% pacienţi. Administrarea warfarinei pe parcursul unei monitorizări stricte de 3 luni au continuat 82,7% persoane, la 6 luni – 67,9%, iar la sfârşitul cercetării (peste 24,87±0,47 luni) doar 48,1% bolnavi administrau ACO, dintre care valoarea optimă a INR (2,0-3,0) au menţinut 29,6% pacienţi. Timpul necesar pentru atingerea unui nivel optim al INR (2,0-3,0) a fost de 1 săptămână. Circa 52% pacienţi nu au primit tratament anticoagulant necesar, motivele suspendării fiind diverse. Concluzii: utilizarea ACO în scopul profilaxiei complicaţiilor tromboembolice la bolnavii cu FA rămâne a fi deficitară, motivele fiind multiple.
Objective: to analyze peculiarities and limitations of indirect anticoagulants administration in patients with atrial fibrillation (AF) and high thromboembolic risk. Methods: prospective study of 81 patients with non-valvular AF (persistent or chronic) and high thromboembolic risk (CHADS 2≥2). Results: Patients’ average age was 59.8±0.84 years, 60.5% being male. 56.8% had a chronic form of AF and 43.2%- a persistent AF. The patients presented the following CHADS 2 scores: 72.8%-2 points, 14.8%-3 points, 8.6%-4 points and 3 patients have had 5 points. 82.7% of the entire sample continued warfarine administration after 3 months, 67.9 %-after 6 months, and by the end of study (after 24.87±0.47 months) only 48.1% of patients were taking oral anticoagulants, of which 29.6% have been maintaining optimal values of INR (2.0-3.0). One week was the time needed for reaching an optimal INR level. About 52% of patients have not received the necessary anticoagulation treatment for various reasons. Conclusions: Anticoagulation use for stroke prevention in AF is still deficient for multiple reasons.
