Deconectarea azygo-portală transabdominală în tratamentul flebectaziilor esofagiene şi gastrice

Abstract

Autorii prezintă rezultatele tratamentului chirurgical al pacienţilor cu hipertensiune portală şi varice esofago-gastrice. În studiu au fost incluşi 47 pacienţi, deconectare azygo-portală limitată (n=7) (operaţia Tanner în modificarea Romero-Torres) şi deconectare azygo-portală extinsă (n=40) (procedee Hassab-Paquet şi Sugiura – Futagawa). S-a demonstrat că intervenţiile de deconectare azygo-portală extinse sunt efective în controlul de durată a varicelor esofagiene şi gastrice, şi pot fi considerate drept intervenţii de elecţie pentru pacienţii cu rezervă funcţională hepatică păstrată (Child-Pugh A).

The authors describe the results of surgical treatment in patients with portal hypertension, oesophageal and gastric varices. Forty seven patients were selected, 7 with limited azygo-portal disconnection (the Romero-Torres modification of the Tanner procedure) and 40 with extended azygo-portal disconnection (Hassab-Paquet and Sugiura – Futagawa procedures). It was demonstrated that extended azygoportal procedures are effective for long-term eradication of oesophageal and gastric varices, and must be considered as elective surgery for the patients with preserved hepatic function (Child-Pugh A).
