AbstractIntroducere: Adenomele papilei Vateri (AA) sunt rare. Malignizarea se înregistrează în 15-30% cazuri. Adenomul vilos este cel mai cancerogen. Opţiunile chirurgicale sunt: tehnici de rezecţie şi ablaţie endoscopică, ampulectomia transduodenală şi pancreatoduodenectomia. Scopul studiului: Evaluarea eficacităţii şi siguranţei papilectomiei endoscopice (PE). Materiale & metode: 12 pacienţi cu AA (5 F: 7 B; intervalul de vârstă 37–68 ani) selectați pentru PE. Sfincterotomie pe coledoc (6), wirsungotomie (3). Stentare biliară (2), wirsungiană (10). Rezultate: Rezecţie en-bloc în 8 cazuri, pe fragmente în 4 cazuri. Adjuvant terapie ablativă 1 caz. Rezecţie completă R0 – 10 cazuri. Tratament chirurgical adjuvant 2 cazuri. Histologic: adenom tubulovilos (5); adenom vilos (4), adenom tubular (2), adenocarcinom (1). Complicaţii: imediate – hemoragie (2), pancreatită (1). Endoscopie la distanţă - stenoze ductale şi recidivă tumorală nu s-a constatat. Concluzii: Papilectoma endoscopică este o sigură alternativă la tratamentul chirurgical al AA.
Summary Adenomas of the duodenal papilla are rare. The frequency of malignant adenomas is 15-30%. Villous adenomas is a premalignant lesion with the highest rate of transformation. Options for surgical treatment include endoscopic and ablation resection, transduodenal ampullectomy, pancreaticoduodenectomy. Aim: Evaluating the efficacy and safety of endoscopic papilectomy for ampullary adenomas (AA). Matherial & Methods: 12 patients (F:M, 5:7, age range 37 - 68 years) with AA were treated by endoscopic papillectomy. BiliarySphincterotomy - 6 cases; pancreatic sphincterotomy - 3. Stenting biliary 2 cases; wirsungy 11 cases. Results: Complete resection R0 - 10 cases. Histopathological: tubulovillous adenomas (5); villous adenomas (4), tubular adenomas (2), adenocarcinoma (1). Complication: bleeding (2) and pancreatitis (1). Follow-up endoscopy - ductal stenosis and recurrence was zero. Conclusion: Endoscopical papillectomy is a safe and well-tolerated alternative to surgical treatment of AA.
