Tulburările hemostazei la bolnavii cu sindrom de leziune pulmonară acută/detresă respiratorie acută (LPA/SDRA)

Abstract

Sindromul de detresă respiratorie acută, în ultimele trei decenii, a devenit una din problemele prioritare ale medicinii şi o mare provocare terapeutică pentru specialistul de anestezie-terapie intensivă. Axarea atenţiei asupra acestei patologii respiratorii s-a datorat nu numai din cauza evoluţiei fulminante a insuficienţei pulmonare, ci şi în urma observaţiilor care arătau că suferinzii de patologia dată de cele mai multe ori aveau şi dereglări ale altor organe şi sisteme, care în rezultat generau evoluţii dramatice ale stării bolnavului şi, implicit, duceau la creşterea costurilor de tratament şi, în cel mai rău caz, se soldau cu rezultate nefavorabile. În această lucrare au fost studiate devierile în sistemul hemostazic la bolnavii cu sindrom de leziune pulmonară acută/detresă respiratorie acută. Studiul a fost efectuat pe un lot de 626 bolnavi cu LPA/SDRA. Bolnavii au fost împărţiţi în trei loturi, în funcţie de leziunile pulmonare pe care le prezentau: uşoare, moderate şi severe. În articol sunt redate modificările în sistemul hemostazic şi sunt analizate cauzele acestor dereglări.

The acute respiratory distress syndrome has become, in the last three decades, one of the medicine’ s foreground problems and a great therapeutic provocation for the specialist of intensive anesthesia-therapy. The centering of attention on this respiratory pathology was due not only to the fulminant evolution of the pulmonary insufficiency, but also to observations that the persons afflicted with the given pathology are frequently suffering from additional disorders of other organs and systems that can precipitate dramatic evolutions of the patient’ s condition with the increase of treatment costs and unfavorable results. In this work have been studied the haemostatic disturbances at the patients with acute lung injury/acute respiratory distress syndrome (ALI/ARDS). The investigations have been carried out on 626 patients with ALI/ARDS. The patients were divided in 3 groups: mild, moderate and severe lung injury. In the article are presented haemostatic modifications and are analyzed the causes of these disturbances.
