Abstract: Herniile Spiegel, cunoscute şi sub denumirea de “hernii ventrale spontane laterale”, reprezintă defecte parietale rare ale liniei semilunare. Raritatea acestui tip de hernie, precum şi unele particularităţi clinico-evolutive şi diagnostice ale unui caz aparte de hernie spigheliană stangulată, i-au determinat pe autori să prezinte această observaţie. Bolnavul C., aproximativ de 35 de ani, originar dintr-o provincie a Sudanului de Sud, cu subnutriţie avansată suferea de o plagă localizată în regiunea infraombilicală laterală pe stânga. Diagnosticul de hernie Spiegel strangulată a fost stabilit intraoperator, plaga fiind rezultatul unei intervenţii tradiţionale locale. A fost executată rezecţia segmentară a ansei deteriorate cu aplicarea enteroanastomozei şi plastia defectului fascial parietal. Perioada postoperatorie a fost caracterizată de o evoluţie gravă cu dehiscenţa parţială a plăgii, rezolvată prin aplicarea suturilor secundare, bolnavul fiind externat după 27 de zile de spitalizare într-o stare satisfăcătoare.
Spiegel`s Hernias, also known as „spontaneous lateral ventral hernias” are rare parietal defects of the semilunar line. The rarity of this type of hernias as well as certain evolutionary clinical and diagnostic peculiarities of a particular case of strangulated Spiegel`s hernia made the authors present this observation. Patient C, about 35 years old, the native of a South Sudan province, with an advanced stage of malnutrition was suffering from a wound in the left lateral infraumbilical region. The diagnosis of strangulated Spiegel`s hernia was established intraoperatively, the wound being the result of a local traditional intervention. A segmental resection of the damaged loop was performed applying enteroanastomosis and plastic restoration of the fascial parietal defect. Postoperative period was characterized by a severe evolution with partial dehiscence of the wound, resolved by applying secondary sutures. The patient being in a satisfactory condition was discharged after 27 days of hospitalization.
