Îngrijirile paliative în Republica Moldova – realitate şi perspectivă

Abstract

Tendinţa de creştere a numărului de bolnavi cu oncopatologii în Republica Moldova presupune majorarea numărului persoanelor cu nevoi de îngrijiri paliative. Obiectivul studiului constă identificarea problemelor şi a perspectivelor de dezvoltare a asistenţei paliative în ţară. A fost efectuat un studiu descriptiv retrospectiv al documentelor de politici în sănătate, s-a evaluat sistemul de acordare a îngrijirilor paliative la nivelele comunitar şi naţional, accesul la opioide şi instruirea personalului medical. Moldova s-a aliat cerinţelor Rezoluţiei OMS pentru îmbunătăţirea accesului pacienţilor la îngrijiri paliative iniţial prin ONG-uri, iar ulterior prin sectorul public. Cu susţinerea financiară a Fundaţiei „SOROS-Moldova”, a Fundaţiei filantropice „Angelus Moldova” au fost instruite 2054 de persoane. În cadrul Proiectului „Fortificarea Capacităţilor de Instruire în Îngrijiri Paliative”, au fost pregătiţii 12 formatori din USMF „N. Testemiţanu” şi instruiţi 120 medici de familie (2016–2017). Accesibilitatea medicamentelor opioide în ţară este asigurată de 190 instituţii autorizate. Moldova dispune de cadrul legislativ pentru integrarea serviciilor de îngrijiri paliative în sistemul de sănătate. Este necesar de a fi continuată pregătirea profesioniştilor şi asigurarea cu preparate opioide conform necesităţilor.

In the Republic of Moldova there is a tendency of increase in the number of patients with oncological diseases, which means a constant rise of persons with palliative care needs. The objective of the study is to identify the problems and perspectives for the development of palliative care in the country. A retrospective, descriptive study of health policy documents, evaluation of palliative care system in the community and at the national level, and training of medical staff in this field were made. The Republic of Moldova has joined the requirements of the WHO Resolution to improve patients’ access to palliative care, initially through NGOs and later through the public health sector. With the financial support from development partners – “Moldova Soros” Foundation, 2051 persons were trained through the Charity Foundation „Angelus Moldova”. Within the framework of the project “Strengthening Palliative Care Capacities”, 12 trainer of trainers from State Medical University of Medicine and Pharmacy “Nicolae Testemitanu” and 120 family doctors were trained in Palliative Care. The availability of opioid drugs in the country is provided by 190 authorized institutions. The Republic of Moldova has the legislative framework for the integration of palliative care services into the health system. The training of professionals in this field is necessary to be continued and sufficient provision of opioids for palliative should be provided.

В Республике Молдова наблюдается тенденция к увеличению числа пациентов с онкопатологией, что означает постоянное увеличение числа лиц, нуждающихся в паллиативной помощи. Было проведено ретроспек тивное описательное исследование документов политики здравоохранения, оценка системы паллиативной помощи на местном и на национальном уровнях, и в области подготовки медицинского персонала. Молдова присоединилась к требованиям Резолюции ВОЗ, чтобы улучшить доступ пациентов к первичной паллиативной помощи через НПО, а позже через государственный сектор. При финансовой поддержке партнеров по развитию, Фонда „СОРОС-Молдова” и Филантропического фонда „Aнгелус Молдова”, 2051 медработников прошли обучение в паллиативной помощи. В рамках проекта «Укрепление потенциала преподавания в сфере паллиативной помощи», Медицинским Университетом подготовлено 12 инструкторов в области паллиативной помощи, которые затем обучили 120 семейных врачей (2016-2017). Опиоидные препараты предоставляются 190 авторизированными учреждениями. Республика Молдова имеет законодательную базу для интеграции услуг паллиативной помощи в систему здравоохранения. Необходимо продолжить подготовку специалистов в этой области, а также обеспечить адекватное предоставление пациентам необходимых опиоидов согласно необходимости.
