AbstractScopul. Ameliorarea managementului chirurgical la pacienți cu plăgi prin arme de foc ale colonului.Material şi metode. Sunt analizate – rezultatele tratamentului chirurgical a 162 de pacienți cu plăgi ale colonului în timpul războiului civil din Republica Angola în anii 1990-1993. Au fost observate complicații postoperatorii la 131 (76,7%) răniți în abdomen cu leziuni ale colonului. Relaparatomii au fost efectuate la 29 (16,9%) răniți: una – la 22, două la 5, trei – la 2 răniți. Majoritatea complicațiilor postoperatorii au apărut în primele 8-10 zile după intervenție chirurgicală. Peritonita postoperatorie s-a dezvoltat la 34 (19,8%) răniți. Printre motivele progresiei peritonitei în primul rînd era asanarea incompletă a cavității abdominale – 15 cazuri (44,1%), în al doilea rînd – dehiscență anastomotică şi a suturilor intestinului subțire – 9 cazuri (26,5%), în al treilea rînd - eşec anastomotic şi a plăgilor colonului – 7 răniți (20,6%). În patru cazuri s-au utilizat relaparotomii programate cu închiderea rănii laparotomice cu fermoar în tratarea finală a peritonitei. Analiza complicațiilor a constatat că au rămas în viață doar acei răniți, la care seria de complicații nu a depăşit 4, iar a cei ce au avut 5 şi mai multe au decedat. La 24 (32,4%) răniți plăgile purulente erau pe fundalul de peritonită progresivă. Din grupul de răniți cu peritonită postoperatorie progresivă au decedat 29 (85,3%) răniți. Sepsisul a fost diagnosticat la 5 răniți (2,9%) cu leziuni mari ale colonului şi ale altor organe abdominale, cu hemoragii masive şi şoc sever. Concluzii. O abordare rațională în ce priveşte complicațiile chirurgicale în comun cu perioada postoperatorie a permis de a reduce mortalitatea la 14,2%.
Aim of the study: Improvement of the surgical management of patients with gunshot lesions ofthe colon. Material and methods: This study includes the analysis of surgical treatment results of 162 patients with colon wounds from the period of the civil war in the Republic of Angola (1990-1993). Postoperative complications were observed at 131 (76,7%) patients with abdominalwounds involving the colon. Relaparotomies were practiced in 29 (16,9%) cases: 22 patients under went one relaparotomy, 5 patients under went two, and two patients – three relaparotomies. Most of the postoperative complications have aroused during the first 8-10 days after surgery. Postoperative peritonitis has developed in 34 (19,8%) cases. The main cause of peritonitis progression was the incomplete lavage of the peritoneal cavity – 15 (44,1%) cases, other causes were: dehiscence of the anastomosis with small intestine leakage – 9 (26,5%) cases, anastomoticinsufficiency and colon wounds leakage – 7 (20,6%) cases. In four cases planned relaparotomies were practiced using zipper systems until complete eradication of the infectious focus. The analysis showed that the patients who had no more than 4 complications have survived, unlike those who had five or more. In 24 (32,4%) cases the purulent wounds were associated to progressive peritonitis. From the group of patients with progressive postoperative peritonitis 29(85,3%) have died. Sepsis has been diagnosed in 5(2,9%) patients, with large lesions of the colon and of other abdominal organs, with massive bleeding and severe shock. Conclusions: A rational approach and management of the surgical complications and postoperative period led to a mortality reduction to 14,2%.
