Abstract: | Background. Breast carcinoma is one of the most common causes of death among women. The factors
that support metastasis are not completely elucidated. Most of women with advanced breast cancer have
regional nodal metastases and distant metastases. Data about lymphangiogenesis in the tumor stroma
are scattered. Objective of the study. Synthesis of bibliographic data to determine the molecular factors
involved in the process of lymphangiogenesis of breast carcinoma. Material and Methods. The study
is based on the analysis of bibliographic sources published in scientific databases in the country and
abroad in the period 2010-2018. Results. Lymphovascular invasion was detected in hematoxylin-eosin
stained sections in 13,8-16% of cases of breast cancer and 28,5% by immunohistochemistry with
markers for Podoplanin. VEGF-C expression correlated with lymphatic vessel density in stroma of
invasive breast carcinomas associated with nodal metastases. VEGF-C was expressed in 64,58% of
cases. Positive expression of VEGF-C, VEGFR-3, and D2-40 was found in HER2 and Luminal B types
of breast carcinoma, associated with frequent metastasis. A significant correlation was reported between
the number of positive peritumoral D2-40 vessels and the density of intratumoral lymphatic vessels
(PCC-0.63; p<0.001). Conclusion. VEGF-C, VEGFR-3, D2-40 and Prox-1 are lymphangiogenic
markers expressed in the stroma of breast carcinomas. These data provide evidence for tumor
lymphangiogenesis and emphasize their diagnostic importance.
Introducere. Carcinomul mamar este cea mai frecventă cauză a decesului la femei. Factorii ce susțin
metastazarea nu sunt complet elucidați. Majoritatea femeilor cu cancer mamar avansat au metastaze
nodale regionale și metastaze la distanță. Datele privind limfangiogeneza în stroma tumorală rămân a fi
controversate. Scopul lucrării. Sinteza datelor bibliografice în vederea determinării factorilor
moleculari implicați în procesul de limfangiogeneză a carcinomului mamar. Material și Metode.
Studiul este bazat pe analiza surselor bibliografice publicate în baze de date știițifice din țară și de peste
hotare în perioada 2010-2018. Rezultate. Invazia limfovasculară a fost detectată în 13,8-16% din
cazurile de carcinom mamar colorate cu hematoxilină-eozină și 28,5% prin imunohistochimie cu
markeri pentru Podoplanină. Expresia VEGF-C corelează cu densitatea vaselor limfatice în stroma
carcinoamele mamare invazive asociate cu metastaze nodale. VEGF-C este exprimat în 64,58% din
cazuri. Expresia pozitivă a VEGF-C, VEGFR-3 și D2-40 s-a constatat în tipurile HER2 și luminal B de
carcinom mamar, asociate cu metastazare frecventă. O corelație semnificativă a fost semnalată dintre
numărul vaselor D2-40 pozitive peritumorale cu densitatea vaselor limfatice intra-tumorale (PCC-0.51;
p<0.001). Concluzii. VEGF-C, VEGFR-3 și D2-40, Prox-1 reprezintă markeri limfangiogenici
exprimați în stroma carcinoamelor mamare invazive. Aceste date probează existența limfangiogenezei
tumorale și accentuează importanța lor diagnostică. |
