Abstract: | Summary.
Clinical and laboratory features and new approaches to
diagnosis and treatment of ulcerative colitis.
The aim of the study was to analyze demographic, clinical
and biological features of ulcerative colitis and improve
diagnostic and treatment schemes based on the risk factors
of unfavorable evolution of the disease. Prospective
cohort study included: 306 patients with ulcerative colitis
with follow-up more than 5 years, 172 – more than 10 years
and 79 – more than 15 years. Controlled clinical studies
were performed for the comparison of different methods of
diagnosis and treatment.
For the fi rst time were evaluated epidemiological trends
and risk factors of ulcerative colitis in Moldova. Patterns of
disease and risk factors for an unfavorable evolution were
identifi ed. There was shown a possibility to predict disease
evolution. A new method of complete remission diagnosis
(by evaluation of markers of the oxidative stress) and a new
tactic of maintenance therapy (intermittent treatment) were
developed. The most signifi cant risk factors: duration until
the specific therapy, therapy at disease onset, duration of
remission are modifiable factors, therefore, the adequate
and timely treatment can improve the disease prognosis. As
a result of integrated approach were developed: classifi cation
of disease evolution patterns and treatment algorithm
based on the risk factors. Резюме.
Клинико-параклинические особенности и новые подходы к диагностике и лечению язвенного колита.
Цель работы – изучить демографические, клинические
и параклинические особенности язвенного колита и усовершенствовать схемы диагностики и лечения в зависимости от факторов риска неблагоприятного течения
заболевания. Проспективное когортное исследование
включало: 306 пациентов с длительностью наблюдения 5 лет, 172 – 10 лет и 79 больных, наблюдаемых в
течение 15 лет. Для сравнения различных методов диагностики и лечения были проведены контролируемые
клинические исследования.
Впервые оценены эпидемиологические тенденции и
факторы риска возникновения язвенного колита в Р.
Молдова. Определены закономерности течения заболевания и факторы риска неблагоприятной эволюции и показана возможность прогнозирования эволюции
заболевания. Разработаны новая методика диагностики полной ремиссии заболевания (методом оценки
маркеров оксидативного стресса) и новая тактика
поддерживающей терапии (интермитирующая поддерживающая терапия). Разработаны классификация
вариантов течения язвенного колита и алгоритм ведения в зависимости от факторов риска. |
