Abstract: | Summary. The purpose of research. Establishing the frequency of herpes infections and clinical and immunological Indicators IRA
at children whose parents got infected with CMV or HSV
Materials and methods. The researched group was formed of 54 children with fatal severe acute respiratory infections,
which were hospitalized in the respiratory diseases section of IMSP SCMC Nr. 1 within the period 2004 to 2009. Patients
were divided into three groups according to age: I-patients aged 6 months -12 months (18.8%), II - 12 to 24 months (62.9%)
-, III – 24 to 36 months (18.3 %). Children have been subjected to clinical-laboratory complex exploration. Herpes infection diagnosis was confirmed by PRC and humoral immunity methods. Mancini method was used to test immunoassay;
cellular immunity was tested with ram erythrocytes. All patients with family history on herpes infections epidemic were
found positive. Results’ analysis was conducted using the statistical method.
Conclusions:
1. Parents with herpes infections present a serious risk infection for their children: all patients with severe evolution of
the IRA whose parents were found with CMV or HSV infections, herpes infection was confirmed by PCR diagnosis to 82%
of the cases and by analyzing the immunity-frmenti in12% of the cases.
2. Acute respiratory infections associated with acute or persistent herpes infections at children, is characterized by severe
progression of the disease, and manifested by acute pneumonia, complicated with cardiomyopathy and toxic-ifectioase
encephalopathies, pancreatitis and severe reactive medium severity.
3. Mixed types of herpes infections (HSV + CMV) were found in most investigated patients: 1 / 3 of the studied children
developed acute infections and 2 / 3 had persistent herpes infections.
4. Acute infections and intracellular herpes affected 2 / 3 of cellular immunity cases (decreasing levels of CD 8, CD 4,
CD 20 and the humor IgA and IgG). Резюме. Цель исследования. Определить частоту герпинфекций и клинико-иммунологические показатели у детей с ОРИ,
родители которых перенесли инфекции CMV, HSV.
Материал и методы. Группу исследованных детей составили 54 ребенка с тяжелым течением ОРИ, которые
были госпитализированы в ГДКБ 1 в 2004-2009 гг. По возрасту дети были распределены на 3 группы: I - дети в
возрасте 6-12 месяцев (18,8%); II – 12- 24 месяцев (62,9%); III - 24-36 месяцев (18,3%). Пациентам было проведено
комплексное клинико-инструментальное обследование. Диагноз герпинфекций был подтвержден методами ПЦР
и ИФА. Гуморальный иммунитет определен методом Mancini, клеточный иммунитет - с помощью эритроцитов
барана. У всех пациентов установлен положительный эпидемиологический семейный анамнез по герпинфекциям.
Анализ результатов исследования проведен с помощью методов медицинской статистики.
Выводы:
1. Наличие герпетических инфекций у родителей представляет высокий риск для инфицирования детей. У
всех детей с тяжелым течением ОРИ, родители которых перенесли CMV или HSV инфекции, подтверждены
герпинфекции – методом ПЦР у 82% больных, ИФА – у 12%.
2.ОРИ, протекающие на фоне герпетических примоинфекций или персистирующих форм, характеризовались
тяжелым течением: осложненные формы пневмоний с ДН 1 степени, кардиомиопатией и энцефалопатией
инфекционно-токсического генеза, реактивным панкреатитом.
3.Смешанная форма герпетической инфекции (HSV+CMV) установлена у большинства обсcледованных детей:
1/3 из них имели примо-инфекцию и 2/3 – персистирующую форму.
4.Сочетание ОРИ с HSV+CMV инфекциями способстовало возникновению вторичного иммунодефицита в 2/3
случаев (понижение уровня CD-8, CD-4, CD-20, сывороточных фракций IgA и IgG). |
