Percepţia aranjamentelor de administrare a asigurărilor obligatorii de asistenţă medicală în Republica Moldova

Abstract

In order to fine-tune the management process of the mandatory health care insurance system in the Republic of Moldova, an insight survey of mandatory social healthcare insurance scheme was carried out. As a tool in this respect, the methodology of key people’s confidence was used. The confidence reveals the degree of trust the decision-makers or consumers give to the current situation or future perspectives of different programs, projects and interventions with social or economic aspects. The data related to the aspects of management in mandatory health care insurance were collected based on a questionnaire developed for this very purpose. The questionnaires were fulfilled only by administration members from local and central public authorities, patronages, syndicates and medical institutions. The interview was carried out during September - October 2008 on a sample size of 451 management staff. The survey showed that the representatives of patronages and syndicates are rather not fully informed about the main characteristics of the social health insurance systems. Also, the key people are rather not satisfied with the process of negotiation between the National Health Insurance Company and the healthcare institutions, in terms of amount, costs and quality of services. The interviewed people have a relatively positive insight on the current setting of centralized management of mandatory healthcare insurance funds. However, this subject shows a tendency towards a neutral attitude, a fact that proves the presence of many negative opinions regarding the centralized management of mandatory healthcare insurance funds. In order to reduce the pressure on the mandatory healthcare insurance funds, exercised by unreimbursed costs, the participants to the survey are confident that there should be established a regime of mandatory insurance for employees at their own businesses (farmers, patent owners), with a set of benefits covering at least the major risks. The survey showed a mutual practical insight of all participants on the need to extend the right to choose the healthcare provider. The key people are very much confident in their agreement to provide the opportunity for the patient to choose freely the specialists in polyclinics or consulting sections of the republican, municipal and district hospitals, as well as from hospital itself. The survey showed that in the Republic of Moldova, the politicians (in its broad meaning) are concerned with the sustainability of mandatory healthcare insurance system. The survey disclosed the fact that the key people are very much confident in supporting the mandatory healthcare insurance system in the future. It identified that the key people would choose to maintain a single common national fund of mandatory healthcare insurance rather than to plead for introducing the treasury concept of financial transactions of the contracted healthcare institutions.

В целях усовершенствования процессов управления проведено исследование по изучению восприятия механизмов администрирования социального медицинского страхования в Республике Молдова. Данные об аспектах управления обязательного медицинского страхования были собраны при помощи специальной анкеты. Анкетирование проведено только среди ключевых лиц, занимающиx высокие должности в центральной и местной администрации, патронатах, профсоюзах и медицинских учреждениях. В результате изучения мнения ключевых лиц было выяснено общее доверие, качество корпоративных переговоров, соблюдение принципа свободного выбора поставщика и устойчивости системы обязательного медицинского страхования в Республике Молдова. Исследование проведено в сентябре-октябре 2008 года на выборке из 451 управленцев. В результате исследования доказано, что представители патронатов и профсоюзов скорее всего не полностью осведомлены об основных особенностях систем социального медицинского страхования. Также, ключевые лица скорее не удовлетворены переговорным процессом на этапе заключения договоров между медицинскими учреждениями и Национальной Компанией Медицинского Страхования, особенно в отношении объемов услуг, их стоимости и качества. Ключевые лица положительно относятся к централизованному способу управления системой обязательного медицинского страхования, но общее мнение приближается к нейтральному уровню, a это свидетельствует о том, что многие не согласны с централизованным управлением фондов обязательного страхования. Для предотвращения случаев неоплаты медицинских услуг предложено ввести отдельные минимальные программы обязательного страхования, особенно для индивидуальных частных предпринимателей. Исследование доказало, что почти все ключевые лица ратуют за расширение прав застрахованных по выбору поставщиков медицинских услуг. Они твердо уверены в том, что пациенты должны свободно выбирать больницу и врачей-специалистов в поликлиниках. Ключевые лица считают, что система обязательного медицинского страхования будет поддержана и в дальнейшем.
