Abstract: Rezumat. Osteonecroza maxilarelor indusă de bisfosfonaţi reprezintă o patologie a cavităţii bucale, caracterizată prin prezenţa ulceraţiilor, expunerea ţesutului osos necrotic şi a complicaţiilor secundare. Scopul: Evaluarea eficacităţii măsurilor de prevenţie şi a planului de tratament al osteonecrozei maxilarelor indusă de bisfosfonaţi, la pacienţii cu osteoporoză. Material şi metode: În studiu sunt 3 paciente DE 65, LV85 şi respectiv VM 73 de ani, cărora li s-au administrat bisfosfonaţi, dintre care la două dintre ele — pe cale orală şi una — intravenos. Pacientele de sex feminin cu vârstele de DE-65 ani; LV-85 ani si VM- 73 ani. Au fost efectuate următoarele investigaţii: examenul clinic, radiologic (OPG si CBCT), osteodensitometria şi analiza markerului osos beta cross-laps, precum şi consultaţia medicului reumatolog. Rezultate: Examinarea şi tratarea pacienţilor a fost în 2 cazuri, fiind sub supraveghere în medie timp de 1 an. Markerul osos betacross-laps a constituit respectiv: DE-0,26 ng/mL, LV-0,22 ng/mL, VM- 0,24 ng/mL. Examenul radiologic a scos în evidenţă semne de sinusită, margini osoase neregulate, porozitate sporită şi într-un caz clinic — detaşarea fragmentului osos. S-a realizat tratamentul medicamentos şi chirurgical, asanându-se cavitatea bucală. Concluzie: Depistarea precoce a osteonecrozei indusă de bisfosfonaţi, precum şi colaborarea interdisciplinară cu medicul reumatolog, determină o micşorare a apariţiei complicaţiilor secundare.
Summary. Bisphosphonate–induced maxillary osteonecrosis represents a pathology of the oral cavity characterized by the presence of ulcerations, exposure of necrotic bone tissue and secondary complications. Purpose: to evaluate the effectiveness of preventive measures and treatment plan of bisphosphonate–induced maxillary osteonecrosis in patients with osteoporosis. Material and methods: In the study there are 3 female patients — DE — 65, LV — 85 and VM — 73 years of age respectively, to whom bisphosphonates were administered, two of them — orally and one — intravenously. The following investigations were performed: clinical, radiological examination (OPG and CBCT), osteodensitometry and beta–cross–laps bone marker analysis, and rheumatologist consultation. Results: Examination and treatment of patients was performed in 2 cases, with follow up for 1 year on average. The beta–cross–laps bone marker was respectively: DE–0.26 ng/mL, LV–0.22 ng/mL, VM–0.24 ng/ mL. Radiological examination revealed signs of sinusitis, irregular bone margins, increased porosity and in one clinical case — bone fragment detachment. Medicinal and surgical treatment was carried out for issues in the oral cavity. Conclusion: Early detection of bisphosphonate–induced osteonecrosis, as well as interdisciplinary collaboration with the physician rheumatologist, results in a decrease in the occurrence of secondary complications.
