Abstract: Introducere. Sclerodermia sistemică este o maladie auto-imună complexă, care se manifestă prin afectarea difuză a ţesutului conjunctiv, cu un tablou clinic variat. Patogenia bolii rămâne a fi o enigmă, deoarece nu este complet elucidată, însă sistemul complementului se crede că ar putea fi implicat în aceasta. Scop. De a determina necesitatea dozării fracţiilor complementului şi interdependenţa dintre valorile fracţiilor complementului (C3 şi C4) şi activitatea bolii, la pacienţii cu sclerodermie sistemică. Material şi metode. În cadrul studiului au fost investigaţi 112 pacienţi, dintre care 106 femei şi 6 bărbaţi, cu vârsta medie 52.3±1.78 ani. 70 pacienţi cu forma limitată a sclerodermiei şi 42 - forma difuză. Pentru evaluarea activităţii bolii, a fost utilizat scorul SCTC-AI. Au fost investigate valorile fracţiilor complementului (C3 şi C4). Rezultate. În urma examinării pacienţilor cu sclerodermie sistemică, au fost identificaţi 2 pacienţi (1,79%), care au avut nivel scăzut a fracţiilor complementului (C3 şi C4), activitatea bolii acestora fiind 1 (activitate joasă) şi ambii au avut sclerodermie sistemică forma limitată. Însă nivel crescut de C3 şi C4 a fost determinat doar la un pacient (0,89%), activitatea bolii a acestuia fiind la fel joasa - 3, conform scorului SCTC-AI. Iar valori normale ale fracţiilor complementului (C3 şi C4) au fost determinate la ceilalţi pacienţi, deci 109 pacienţi (97,32%), a caror activitatea bolii a fost medie: 7.2±1.4, conform scorului SCTC-AI. Concluzii. În urma efectuării studiului am determinat că nu este o interdependenţă între valorile fracţiilor complementului (C3 şi C4) cu activitatea bolii la pacienţii cu sclero-dermie sistemică. Astfel dozarea C3 şi C4 nu reprezintă un marker pentru aprecierea activităţii bolii (sclerodermie sistemică).
Introduction. Systemic sclerosis is an autoimmune disease characterized by diffuse connective tissue involvement, presenting with a varied clinical picture. The pathogenesis of the disease remains an enigma, it is not fully understood, but the complement system is believed to play a potential role in its development. Objective. To determine the necessity of measuring complement fraction and the interdependence between complement fraction levels (C3 and C4) and disease activity in patients with systemic scleroderma. Material and methods. The study included 112 patients, of whom 106 were women and 6 were men, with a mean age of 52.3 ± 1.78 years. Among them, 70 patients had the limited form of scleroderma and 42 had the diffuse form. Disease activity was assessed using the SCTC-AI score. Complement component levels (C3 and C4) were also evaluated for the patients. Results. Following the examination of patients with systemic scleroderma, two patients (1.79%) were identified with decreased levels of complement components (C3 and C4); both had low disease activity (score of 1) and were diagnosed with the limited form of systemic scleroderma. Elevated levels of C3 and C4 were found in only one patient (0.89%), whose disease activity was also low (score of 3) according to the SCTC-AI. Normal complement component levels (C3 and C4) were observed in the remaining 109 patients (97.32%), with an average disease activity classified as moderate: 7.2 ± 1.4, based on the SCTC-AI score. Conclusion. Based on the results of the study, no interdependence was found between complement component levels (C3 and C4) and disease activity in patients with systemic scleroderma. So, the measurement of C3 and C4 does not appear to serve as a reliable marker for assessing disease activity.
