Abstract: Introducere. Boala cronică de rinichi (BCR) este o patologie cu evoluţie progresivă şi ireversibilă, ce afectează calitatea vieţii încă din fazele incipiente ale bolii. Simptome precum fatigabilitatea, durerea persistentă şi tulburările de somn influenţează negativ starea fizică şi psihică a pacienţilor. Scop. Evaluarea relaţiei dintre severitatea simptomelor şi nivelul calităţii vieţii la pacienţii cu boală cronică de rinichi, în funcţie de stadiul evolutiv al afecţiunii. Material şi metode. S-a efectuat un studiu transversal care a inclus 989 pacienţi cu boală cronică de rinichi în stadiile I-V până la tratamentul prin hemodializă, internaţi timp de un an, în Secţia de Nefrologie a SCR „Timofei Moşneaga”. Calitatea vieţii a fost evaluată prin chestionarul validat KDQOL-SF™ 1.3, specific pentru pacienţii cu BCR. Rezultate. Din lotul pacienţilor selectaţi 64,5% au fost femei şi 35,5% - bărbaţi, vârsta medie pentru ambele sexe a fost de 57,7±13,1 ani. Repartizarea după stadiul BCR stadiul I - 31,0%, stadiul II - 26.1%, stadiul III - 19.7%, stadiul - IV 15.3%, stadiul V - 7.9%. Cele mai frecvente manifestări clinice au fost: astenia fizică (91.7%), lombalgiile (91.5%), dizuria (64.8%), prezenţa edemelor (51.9%), disfuncţie sexuală (32.1%), insomnie (52.2%). Valoarea medie ai simptomelor renale asupra calităţii vieţii a fost 63.6% (P < 0,0001), indicând o corelaţie puternică între severitatea simptomelor şi deteriorarea calităţii vieţii pacienţilor. Concluzii. Simptomele severe asociate bolii cronice de rinichi au un impact negativ important asupra calităţii vieţii pacienţilor. Recunoaşterea precoce şi gestionarea eficientă a acestora sunt esenţiale pentru susţinerea calităţii vieţii şi a funcţionării zilnice în evoluţia bolii.
Introduction. Chronic Kidney Disease (CKD) is a progressive and irreversible disease that affects quality of life in its early stages. Symptoms such as persistent fatigue, chronic pain and sleep disturbances have a negative impact on physical health and mental well-being, reducing patients’ overall daily functioning. Objective. Examining the connection between symptom intensity and quality of life in patients with chronic kidney disease, based on the stage of illness, to evaluate the impact of disease progression. Material and methods. We conducted a cross-sectional study including 989 patients with chronic kidney disease in stages I-V until hemodialysis treatment, hospitalized for one year in the Nephrology Ward of Timofei Mosneaga RCH. Quality of life was assessed using the validated KDQOL-SF™ 1.3 questionnaire, specific for patients with BCR. Results. Of the study group, 64.5% were women and 35.5% were men, with an overall mean age of 57.7±13.1 years. The staging of chronic kidney disease was as follows: stage I - 31.0%, stage II - 26.1%, stage III - 19.7%, stage IV - 15.3% and stage V - 7.9%. The most common symptoms experienced were physical exhaustion (91.7%), low back discomfort (91.5%), urinary difficulties (64.8%), fluid retention (51.9%), sleep disturbance (52.2%) and reduced sexual function (32.1%). The mean impact of kidney-related symptoms on quality of life was 63.6% (P < 0.0001), showing a strong association between symptom severity and decreased quality of life. Conclusion. The severe symptoms associated with chronic kidney disease have a significant negative impact on patients’ quality of life. Their early recognition and effective management are essential to sustain quality of life and daily functioning throughout the course of the disease.
