Abstract: Introducere. Infarctul miocardic cu artere coronariene non-obstructive (MINOCA) desemnează o formă de sindrom coronarian acut cu tablou clinic sugestiv, elevarea troponinelor şi stenoză coronariană sub 50%. Inflamaţia vasculară inhibă adipogeneza în ţesutul adipos pericoronar (PCAT) şi poate fi evaluată prin angio-CT. Scop. Studierea modificărilor ţesutului adipos pericoronar la pacienţii cu sindromul MINOCA prin analiza PCAT realizată prin an-gio-CT, metodă eficientă non-invazivă de diagnostic. Material şi metode. Reviul literaturii a fost realizat prin analiza publicaţiilor ştiinţifice catalogate în baze de date electronice: PubMed, Medline, BMJ Journals, Oxford Academic, elaborate în perioada 2020-2025 şi ghidul Societăţii Europene de Cardiologie 2023, privind aplicabilitatea analizei PCAT în diagnosticul inflamaţiei coronariene la bolnavii cu MINOCA. Rezultate. PCAT, biomarker imagistic evaluat prin metoda non-invazivă Angio CT, reflectă modificările fenotipice ale ţesutului adipos perivascular inflamat, asociat cu eliberarea de citokine proinflamatorii şi alterarea componentelor lipidice locale. Valori mai mari de -70HU marcând instabilitatea plăcii aterosclerotice. Corelaţia cu biomarkerii inflamatori circulanţi (CRP, IL 6), susţine valoarea prognostică a PCAT, scorurile crescute asociindu-se cu un risc semnificativ de evenimente cardiovasculare majore (MACE) secundare (Hazard Ratio~ 3) şi mortalitate cardiacă crescută, detectabile în stadii precoce, permiţând optimizarea strategiilor terapeutice. Concluzii. Utilitatea scorului PCAT evaluat în angiografie prin tomografie computerizată contribuie semnificativ la stratificarea riscului pacienţilor cu MINOCA prin cuantificarea inflamaţiei pericoronariene, facilitând deciziile terapeutice timpurii şi personalizate în practica cardio-logică.
Introduction. Myocardial infarction with non-obstructive coronary arteries (MINOCA) is characterized by symptoms suggestive of acute coronary syndrome, elevated troponin levels, and coronary stenosis of less than 50%. Vascular inflammation in pericoronary adipose tissue (PCAT) can be assessed using coronary CT angiography. Objective. Investigation of PCAT changes among patients with MINO-CA syndrome using coronary CT angiography, a reliable, non-invasive imaging modality for the assessment of vascular inflammation. Material and methods. A literature review was conducted by analyzing scientific publications indexed in electronic databases (PubMed, Medline, BMJ Journals, and Oxford Academic) published between 2020 and 2025, along with the 2023 ESC guidelines, with a focus on the applicability of PCAT analysis for diagnosing coronary inflammation in patients with MINOCA. Results. PCAT, an imaging biomarker assessed non-invasively by coronary CT angiography, reflects phenotypic changes of inflamed perivascular adipose tissue, associated with the release of proinflammatory cytokines and alterations in local lipid components. Values above -70 HU indicate atherosclerotic plaque instability. The correlation between inflammatory biomarkers (CRP, IL-6) supports the prognostic value of PCAT, and higher scores are associated with a significantly increased risk of major cardiovascular events - MACE (Hazard Ratio ~ 3) and elevated cardiac mortality, detectable at early stages and allowing optimization of therapeutic strategies. Conclusion. The utility of the PCAT score assessed by coronary computed tomography angiography significantly contributes to risk stratification in MINOCA patients by quantifying pericoronary inflammation, facilitating early and personalized therapeutic decisions in cardiology practice.
