PROBLEMA REZISTENŢEI MICROBIENE ÎN CONTEXTUL UTILIZĂRII DEZINFECTANŢILOR

Abstract

Introducere. Rezistenţa la antimicrobiene reprezintă una dintre cele mai mari ameninţări la adresa sănătăţii publice la nivel global. Pe lângă utilizarea neraţională a antibioticelor, un factor subestimat este utilizarea pe larg a dezin-fectanţilor. Acest fenomen conduce la rezistenţă microorganismelor la dezinfectante. Scop. Constatarea problemei rezistentei microbiene faţă de dezinfectante la etapa contemporană. Material şi metode. A fost realizat un studiu al surselor bibliografice ştiinţifice care abordează problema rezistenţei microbiene faţă de dezinfectante. Rezultate. Potrivit unui raport al European Centre for Disease Prevention and Control s-a constatat că Quaternary Ammonium Compounds, utilizaţi pe scară largă, au fost corelaţi cu apariţia tulpinilor de P. aeruginosa şi S. aureus rezistente la antimicrobiene. Studiile recente au arătat că expunerea repetată la clorhexidină induce expresia genelor de eflux. Utilizarea dezinfectanţilor cu alcool izopropilic a condus la apariţia unor tulpini de E. faecium cu toleranţă la alcool. Conform rapoartelor ANSP, s-a observat o creştere semnificativă a utilizării dezinfectantelor şi izolatelor bacteriene multirezistente în spitale (ex. K. pneumoniae, A. baumannii). Concluzii. Lipsa implementării unui monitoring al rezistenţei microorganismelor faţă de dezinfectante la nivel de instituţie medicală duce la selecţia microbiană rezistentă şi pierderea eficienţei antimicrobienelor, cu impact major asupra sănătăţii publice şi economiei.

Introduction. Antimicrobial resistance is one of the biggest threats to public health globally. In addition to the irrational use of antibiotics, an underestimated factor is the widespread use of disinfectants. This phenomenon leads to the resistance of microorganisms to disinfectants. Objective. Finding the problem of microbial resistance to disinfectants at the contemporary stage. Material and methods. A study of scientific bibliographic sources addressing the issue of microbial resistance to disinfectants was carried out. Results. According to a report by the European Center for Disease Prevention and Control, widely used Quaternary Ammonium Compounds have been linked to the emergence of antimicrobial-resistant strains of P. aeruginosa and S. aureus. Recent studies have shown that repeated exposure to chlorhexidine induces the expression of efflux genes. The use of isopropyl alcohol disinfectants has led to the emergence of alcohol-tolerant strains of E. faecium. According to ANSP reports, there has been a significant increase in the use of disinfectants and multi-resistant bacterial isolates in hospitals (eg K. pneumoniae, A. baumannii). Conclusion. The lack of implementation of a monitoring of the resistance of microorganisms to disinfectants at the medical institution level leads to the selection of resistant microbes and the loss of effectiveness of antimicrobials, with a major impact on public health and the economy.
