RINOFIMA. DIAGNOSTIC ŞI TRATAMENT, CAZ CLINIC

Abstract

Introducere. Rinofima o formă avansată de rozacee, caracterizată prin hipertrofia progresivă a glandelor sebacee şi fibroză la nivelul nasului. Se manifestă prin deformări inestetice deformante. Deşi benignă, rinofima necesită tratament chirurgical în formele avansate pentru restabilirea funcţiei şi a aspectului estetic. Scop. Determinarea şi evidenţierea particularităţilor de diagnostic, conduită şi tratament, evoluţie şi îngrijire postoperatorie a plăgii prin vindecarea per-secundam în cazul rinofimei. Material şi metode. Pacient A./D., gen masculin, 67 de ani, s-a adresat la IMU, cu aspectul inestetic progresiv al nasului. Clinic s-a evidenţiat o hipertrofie difuză a tegumentului nazal cu aspect nodular, roşeaţă şi pori dilataţi. S-a optat pentru tratamentul chirurgical prin rezecţie parţială a formaţiunii, urmată de remod-elare tisulară. Rezultate. S-a iniţiat intervenţia chirurgicală de rezecţie a ţesutului fibrozant, prin excizia cu bisturiul şi remodelarea cu diatermocoagulatorul şi deramabrazie, utilizând freză din zirconiu. Plaga postoperatorie a fost menajată prin pansamente zilnice şi utilizarea membranelor de colagen. Intervenţia s-a desfăşurat fără de complicaţii intraoperatorii şi postoperatorii imediate. Evoluţia locală a fost favorabilă, cu epitelizarea per secundam şi restabilirea conturului nazal într-un mod fiziologic şi estetic. La control repetat peste 3 luni postoperator, rezultatul a fost considerat stabil şi estetic din punct de vedere clinic şi chirurgical. Concluzii. Tratamentul chirurgical al rinofimei reprezintă soluţia optimă în formele avansate cu deformare estetică severă. Abordarea corectă, instrumentarul adecvat şi îngrijirea postoperatorie contribuie decisiv la succesul tratamentului şi la reintegrarea socială a pacientului.

Introduction. Rhinophyma - advanced form of rosacea, characterized by progressive hypertrophy of the sebaceous glands and fibrosis of the nasal tissue. It manifests as disfiguring and unaesthetic nasal deformities. Being benign, requires surgical treatment in advanced stages to restore function and aesthetic appearance. Objective. Determination and highlighting of the diagnostic, management, and treatment particularities, as well as the postoperative wound care and healing by second intention in a case of rhinophyma. Material and methods. Patient A./D., male, 67 years old, presented to IMU with progressive unaesthetic nasal enlargement. Clinical examination revealed diffuse hypertrophy of the nasal skin with a nodular appearance, redness, and enlarged pores. Surgical treatment was chosen, consisting of partial resection of the lesion followed by tissue remodeling. Results. The surgical intervention for resection of fibrotic tissue was initiated by scalpel excision, followed by remodeling using a diathermy device and dermabrasion with a zirconium bur. The postoperative wound was managed with daily dressings and collagen membrane application. The procedure was completed without intraoperative or immediate postoperative complications. Local evolution was favorable, with second-intention epitheliali-zation and restoration of the nasal contour in a physiologic and aesthetic manner. At the 3-month postoperative follow-up, the outcome was considered stable and aesthetically from clinical and surgical perspectives. Conclusion. Surgical treatment remains the optimal solution in advanced cases of rhinophyma with severe aesthetic deformation. Proper surgical planning, instrumentation, and meticulous postoperative care are essential for achieving therapeutic success and facilitating the patient's social reintegration.
