Abstract: Rezumat Pacienții cu boală hepatică cronică avansată prezintă modi-ficări în toate fazele hemostazei. Dovezile clinice sugerează o legătură între boala hepatică și modificările factorului von Willebrand (VWF), fiind coroborate cu date mecanistice emergente care arată un rol negativ al VWF în progresia bolii hepatice. Nivelurile ridicate de VWF accelerează formarea trombilor intrahepatici și, prin urmare, pot fi un factor deter-minant al progresiei bolii. Prezenta analiză se concentrează pe dovezile clinice și experimentale care susțin o legătură între funcția VWF și progresia bolilor hepatice cronice. A fost efectu-ată o căutare bibliografică cuprinzătoare cu utilizarea bazelor de date PubMed, Scopus și Web of Science pentru articole publicate până la data de 1 iunie 2025. Termenii de căutare au inclus următoarele combinații: „factor von Willebrand”, „boală hepatică”, „ciroză”, „tromboză venoasă portală”. Dez-echilibrul VWF/ADAMTS13 corelează cu severitatea bolii hepatice și cu prognosticul slab în boala hepatică. Factorul von Willebrand este un biomarker neinvaziv promițător al disfuncției endoteliale și al progresiei bolii hepatice, dar și o țintă nouă terapeutică.
Summary The role of Von Willebrand factor in chronic liver diseasePatients with advanced chronic liver disease present alterations in all phases of hemostasis. Clinical evidence suggests a link between liver disease and changes in von Willebrand factor (VWF) which is supported by emerging mechanistic data showing a negative role of VWF in liver disease progression. Elevated VWF levels accelerate intrahepatic thrombus forma-tion and therefore may be a determinant of disease progres-sion. This review focuses on the clinical and experimental evidence supporting a link between VWF function and the progression of chronic liver disease. A comprehensive biblio-graphic search was performed using the PubMed, Scopus, and Web of Science databases for articles published up to June 1, 2025. Search terms included combinations of: “von Willebrand factor”, “liver disease”, “cirrhosis”, “portal vein thrombosis”. VWF/ADAMTS13 imbalance correlates with severity of liver disease and poor prognosis in liver disease. Von Willebrand factor is a promising noninvasive biomarker of endothelial dysfunction and liver disease progression, as well as a novel therapeutic target.
Резюме Роль фактора Виллебранда при хронических заболе-ваниях печениУ пациентов с прогрессирующим хроническим заболе-ванием печени наблюдаются изменения на всех фазах гемостаза. Клинические данные свидетельствуют о связи между заболеванием печени и изменениями фак-тора Виллебранда (VWF), что подтверждается новыми механистическими данными, показывающими неблаго-приятную роль VWF в прогрессировании заболевания печени. Повышенные уровни VWF ускоряют образование внутрипеченочных тромбов и, следовательно, могут быть определяющим фактором прогрессирования за-болевания. В этом обзоре основное внимание уделяется клиническим и экспериментальным данным, подтверж-дающим связь между функцией VWF и прогрессирова-нием хронического заболевания печени. Был проведен комплексный библиографический поиск с использованием баз данных PubMed, Scopus и Web of Science для статей, опубликованных до 1 июня 2025 года. Поисковые терми-ны включали комбинации: «фактор Виллебранда», «за-болевание печени», «цирроз», «тромбоз воротной вены». Дисбаланс VWF/ADAMTS13 коррелирует с тяжестью заболевания печени и плохим прогнозом при заболевании печени. Фактор Виллебранда является перспективным неинвазивным биомаркером эндотелиальной дисфунк-ции и прогрессирования заболевания печени, а также новой терапевтической мишенью.
