Evoluţia clinică în perioada postoperatorie a pacienţilor cu guşă nodulară

Abstract

In this research were analized the clinical aspects of patients with nodular goiter after surgical treatment. It was demonstrated that the criteria for confirmation of the postoperative euthyroidism serve for: the determination of T3, T4, TSH normal limits and no increase in the volume of tissue was registered. The determination of the increase of the TSH level - according to general criteria, most frequently at the lower limit of a total T3 and T4 allows the establishment of the diagnosis of subclinical hypothyroidism and the recommendation of a new treatment. During the two years of study, of the total number of 58 patients, the frequency of euthyroidism decreased slightly and it was 49.8%, the postoperative hypothyroidism was determined in 39.7% case, and 10.4% cases with relapse nodular goiter (the first recorded cases were from 6 to 11 postoperatively months, and it was of 5.1%).

În această lucrare au fost analizate aspectele clinico-paraclinice a pacienţilor cu guşă nodulară după tratamentul chirurgical. S-a demonstrat că criteriile de confirmare a eutiroidiei postoperatorii pot servi: determinarea T3, T4, TSH în limitele normei şi lipsa creşterii în volum a ţesutului restant. În cazul determinării creşterii nivelului TSH - ului pe fonul valorilor normale, mai frecvent la limita inferioară ale T3 şi T4 totali se poate stabili diagnosticul de hipotiroidie subclinică şi poate fi recomandată instituirea tratamentului substitutiv; Pe parcursul a doi ani de studiu, din numărul total de 58 de pacienţi, frecvenţa eutiroidiei a scăzut nesemnificativ şi a constituit 49,8%, hipotiroidia postoperatorie manifestă s-a determinat în 39,7% cazuri, iar recidiva guşii nodulare în 10,4% cazuri (primele cazuri înregistrîndu-se la termenul de 6-11 luni postoperator, şi constituia 5,1%).
