Introducere. Traumatismul pancreatoduodenal (PD) reprezintă unul din cele mai grave traumatisme
abdominale, având ca rezultat dezvoltarea unui lanţ patologic: trauma pancreasului se complică cu pancreatita
acută posttraumatică (PAPT), iar peritonita fermentativă – cu dehiscența suturilor duodenale.
Scop. Aprecierea tacticii medico-chirurgicale în traumatismul PD.
Material și metode. S-au analizat metodele de tratament la 30 pacienți cu traumatism PD. La 23 pts au fost
leziuni ale pancreasului de gr. I şi II, gr. III, IV și V fiind numai la 7. Segmentul D1 a fost lezat la 5 pst, D2- la
15, D3-la 8, D4– la 2 pts. Duodenul a fost exclus din pasaj la 9 pts. Drenarea închisă a bursei omentale (BO) s-a
efectuat la 23 pts., iar aplicarea bursomentostomiei (BOS) - la 7 pts.
Rezultate. PAPT a fost constatată în 75% şi 100% dintre pacienții respectiv cu păstrarea şi excluderea duodenului
din pasaj. BOS s-a aplicat în leziunile traumatice ale pancreasului de gr. III – V, frecvența PAPT fiind 83,3%, cu
o mortalitate de 33,3%, în comparaţie cu gr. I şi II, unde frecvenţa PAPT şi letalităţii a fost de 82,3% şi 23,5%.
Suturarea duodenului fără excludere din pasaj cu aplicarea BOS este motivată de spitalizarea tardivă şi prezenţa
deja a PAPT. La pacienţii cu excluderea duodenului din pasaj frecvenţa complicaţiilor infecțioase şi a letalităţii
a predominat la drenarea închisă a BO: 20% vs 0%; 60% vs 50%. Mortalitatea în perioada postoperatorie tardivă
a fost de 50%.
Concluzii. Tratamentul leziunilor traumatice PD necesită o abordare selectivă în dependență de gradul leziunii
pancreasului, prezența PAPT și gravitatea leziunilor duodenului.

the development of a pathologic chain: pancreatic trauma is compounded with posttraumatic acute pancreatitis
(PTAP) and fermentative peritonitis and in the result - the duodenal suture dehiscence.
Purpose. Assessment of surgical tactics of treatment in PD trauma.
Material and Methods. We analyzed the methods of treatment in 30 patients with severe PD trauma. 23 pts had
lesions of the pancreas gr. I and II; gr. III, IV and V of lesions were found only in 7 patients. Segment D1 has
been damaged at 5 pts; D2 – 15, D3 - 8, D4 - 2 pts. The duodenum was expelled from passage to 9 pts. Closed
drainage of bursa omentalis (BO) was carried out at 23 pts., and the application of burso-omentostomy (BOS)
was performed at 7 pts.
Results. PTAP was found in 75% and 100% of patients respectively with duodenal exclusion from/keeping in is 83.3%, with a mortality rate of 33.3% compared to grade I-II, where the frequency of PTAP and lethality
were 82.3% and 23.5% respectively. Suturing without duodenum exclusion of passage and BOS application has
been indicated in patients with tardive hospitalization and already presence of PTAP. In patients with duodenal
exclusion of passage frequency and lethality of infectious complications prevailed in the closed drainage of BO:
20% vs 0%; 60% vs 50%. Mortality in the late postoperative period was 50%.
Conclusions. Treatment of pancreaticoduodenal trauma requires a selective approach that depends on the
degree of pancreatic injury, presence of PTAP and of the severity of duodenum lesions.
