Tratamentul chirurgical al bolii hemoroidale cu ajutorul aparatului A.M.I. Hal-Rar

Abstract

Introducere. Boala hemoroidala rămâne o problemă actuală. Ligaturarea transanală a arterelor hemoroidale sub controlul dopplerometriei ultrasonore cu mucopexie (HAL-RAR) este o metodă chirurgicală contemporană, patogenetică, miniminvazivă de tratament al hemoroizilor interni. Scopul. Aprecierea eficacității metodei miniminvazive HAL-RAR în tratamentul hemoroizilor. Material și metode. În clinica de chirurgie a Spitalului Clinic Central, stația Chișinău, în anii 2014-2016 au fost efectuate 16 intervenții chirurgicale pacienților cu diagnosticul de hemoroizi cronici micști gr.III-IV cu ajutorul aparatului A.M.I. HAL-RAR. La toți pacienții preoperator a fost efectuată fibrocolonoscopia. În timpul intervenţiei chirurgicale cu ajutorul aparatului A.M.I. HAL-RAR a fost efectuată la 7 pacienţi ligaturarea doplerghidată a ramurilor arterei hemoroidale superioare (HAL). La 9 pacienţi cu prolapsul masiv al mucoasei anale după procedeul HAL a fost efectuată mucopexia (HAL-RAR). Rezultate. Cel mai frecvent ramurile arterei hemoroidale superioare au fost depistate la orele 1, 3, 5, 7, 9, 11. La toţi pacienți după efectuarea metodei HAL-RAR sindromul algic a fost slab pronunțat şi nu a fost necesară administrarea opiodelor. Durata spitalizării a variat de la 3 până la 5 zile. Complicații intra-şi postoperatorii n-au fost depistate. Toți pacienții au fost externați la domiciliu în stare satisfăcătoare. Rezultatele la distanţa până la 12 luni au fost analizate la 11 pacienţi. Recidive a bolii hemoroidale n-au fost depistate. Concluzii. Metoda HAL-RAR este o metoda eficace alternativă în tratamentul chirurgical al hemoroizilor.

Introduction. Hemorrhoidal disease remains an actual problem. Transanal doppler-guided hemorrhoidal artery ligation and recto anal repair (HAL-RAR) is a contemporary, pathogenetic, minimally invasive surgical method for treatment of internal hemorrhoids. Purpose. Assessing the effectiveness of minimally invasive HAL-RAR method in the treatment of hemorrhoids. Materials and methods. In the surgery clinic of the Central Railway Hospital, Chisinau station, in 2014-2016 there were performed 16 surgical interventions in patients diagnosed with chronic mixed hemorrhoids IIIIV grade with the A.M.I. HAL-RAR device. All patients before surgery underwent colonoscopy. In 7 patients during surgery ligation of superior hemorrhoidal artery branches was performed using the A.M.I. HAL-RAR device (HAL). In 9 patients with massive prolapse of anal mucosa after HAL procedure recto anal repair was performed (HAL-RAR). Results. Superior hemorrhoidal artery branches were determined at 1, 3, 5, 7, 9, 11 o’clock most commonly. After the HAL-RAR method a mild pain syndrome was observed in all 16 patients and there was no necessity in opioids prescribing. Hospitalization period ranged from 3 to 5 days. Intraoperative and postoperative complications were not determined. All the patients were discharged home in satisfactory condition. Longterm results up to 12 months were analyzed in 11 patients. Hemorrhoidal disease recurrence was not detected.Conclusions. HAL-RAR is an effective alternative method in surgical treatment of hemorrhoids.
