Chistadenocarcinom mucinos apendicular complicat cu fistulă cutanată spontană

Abstract

Introducere. Mucocelul apendicular este o maladie rară caracterizată prin dilatarea chistică a lumenului apendicular cu acumulare de mucus în interior. Mucocelul apendicular cu fistulă cutanată este extrem de rar, fiind descrise doar patru astfel de cazuri până în prezent. Prezentăm un caz clinic de chistadenocarcinom mucinos apendicular cu fistulă cutanată spontană. Material și metode. Pacienta în vârstă de 80 ani s-a prezentat pentru o ulceraţie cutanată cu eliminări mucinoase în regiunea iliacă dreaptă, timp de o lună. Ecografia abdominală şi examenul CT au pus în evidenţă o formaţiune tumorală chistică cu ecogenitate şi densitate mixtă, având originea în apendice, implicând peretele abdominal, fără semne de ascită ori metastaze. S-a practicat hemicolonectomie dreaptă cu limfadenectomie şi rezecţia fistulei cutanate. Examenul histologic a confirmat chistadenocarcinom apendicular moderat diferenţiat. Rezultate. Evoluţia postoperatorie a fost bună, pacienta fiind externată la 11 zile postoperator. Fiind supravegheată timp de 12 luni pacienta nu a prezentat semne de recidivă a maladiei. Concluzii. Autorii prezintă revista literaturii, modalităţile de diagnostic şi tratament pentru această maladie extrem de rară.

Introduction. Mucocele of the appendix is a relatively rare disease characterized by a cystic dilatation of the appendiceal lumen with stasis of mucus inside. Appendiceal mucocele with a cutaneous fistula is an extremely rare clinical entity, only four such cases being described in the medical literature. We report an additional case of cystadenocarcinoma of the appendix with spontaneous cutaneous fistula. Material & methods. The patient is an 80-year old woman, who originally presented with a skin ulcer in the right lower abdominal quadrant with mucus discharge during the last month. Abdominal ultrasonography and computed tomography demonstrated cystic mass with mixed echogenity and density originating from the appendix, involving the abdominal wall, without signs of ascites or metastasis. A right hemi-colectomy, lymphadenectomy and resection of the skin fistula was performed. The histological findings revealed moderately differentiated mucinous cystadenocarcinoma of the appendix. Results. The postoperative period was uneventful and the patient discharged on the 11th POD. During 12 month follow-up period the patient is free of disease recurrence. Conclusion. The authors present the literature review, diagnostic features and the treatment policy of this extremely rare condition.
