Hemostază reuşită cu utilizarea mini-loop pentru varice duodenale hemoragice: caz clinic

Abstract

Introducere. Hemoragia din varicele duodenale este o manifestare rară şi deseori fatală a hipertensiunii portale, iar aceste varice ectopice sunt mai des observate în cazul ocluziei porţiunii extrahepatice a venei porte. În prezent sunt descrise circa 160 cazuri de varice duodenale în literatura Anglosaxonă. Material și metode. O pacientă de 47 ani a fost internată pentru hematemeză masivă şi şoc. Examenul endoscopic a pus în evidenţă varice esofagiene mici nonhemoragice şi varice hemoragice în D1. Rezultate. A fost efectuată hemostază endoscopică cu 5 ligaturi de nailon. Hipertensiunea portală a fost indusă de ciroza hepatică şi cavernom portal posttrombotic. Concluzii Prezentăm primul caz de ligaturare reuşită cu mini-loop a varicelor duodenale hemoragice.

Introduction. Bleeding from duodenal varices is a rare, but often fatal manifestation of portal hypertension and these ectopic varices are more common in extrahepatic portal venous obstruction. There are over 160 cases of duodenal varices reported in the English literature. Material & methods. A 47-year-old female presented with massive hematemesis and prolonged shock. Initial endoscopy revealed non-bleeding small esophageal varices and large varices in the first portion of the duodenum with spurting bleeding. Results. Endoscopic hemostasis was obtained with 5 detachable nylon loops. Portal hypertension was caused by liver cirrhosis and postthrombotic portal cavernoma. Conclusion. To the best of our knowledge this is the first case of successful mini-loop ligation of bleeding duodenal varices reported in the literature.
