Clasificarea şi algoritmul tratamentului formelor distructive ale erizipelului

Abstract

Introducere. În ultimul deceniu a crescut semnificativ numărul de pacienţi cu forme distructive ale erizipelului(FDE).Lipsa unei clasificări a complicaţiilor locale şi tactici de conduită chirurgicală a acestor forme au contribuit la rezultate nesatisfăcătoare a tratamentului. Scop. Elaborarea clasificării complicaţiilor locale FDE şi algoritmului complex al tratamentului. Material şi metode. Studiul cuprinde 52 de pacienţi cu FDE pe o perioadă de 10 ani (2006-2015). Rezultate. După evoluţia bolii a fost propusă următoarea clasificare a complicaţiilor locale FDE: A. Complicaţii locale minore: a) Pustulizarea bulelor. b) Necroze superficiale limitate ale pielii cu suprafaţa până la10 cm2). c) Abcese mici ale pielii. B. Complicaţii locale majore:a) Necroze profunde ale pielii cu suprafaţa mai mare de 10 cm2 . b) Flegmoane. c) Fasciite. Algoritmul tratamentului FDE: A) În complicaţiile locale minore: Deschiderea bulelor pustulizate şi abceselor mici, excizia necrozelor superficiale limitate ale pielii. B) În complicaţiile locale majore: Debridarea chirurgicală agresivă a focarului necrotic; Revizia şi sanarea zilnică a plăgii sub anestezie generală; Tratamentul plăgii cu betadină, unguente hidrosolubile; Vacuum aspiraţia plăgii; Aplicarea suturilor secundare; Aplicarea aparatului Ilizarov în plăgile circulare; Autodermoplastia; Tratamentul medicamentos: antibioticoterapia, terapia infuzională, desensibilizante, antiagregante, vitaminoterapia, etc; Tratament fizioterapeutic. Concluzii. Clasificarea elaborată a complicaţiilor locale a FDE şi algoritmului complex al tratamentului poate fi folosită de specialişti în practica cotidiană.

Introduction. In the last decade has increased significantly the number of patients with destructive forms of erysipelas (DFE). The absence of classification of local complications and surgical tactics of these forms contributed to unsatisfactory results of treatment. Aim. Elaboration the classification of local complications DFE and the complex algorithm of treatment. Materials and Method. The study includes 52 patients with DFE for a period 10 years (2006-2015). Results. After the evolution of illness it was proposed the next classification of local complications of DFE: A. Minor local complications: a) suppuration bubbles. b) Limited superficial necrosis of skin with the area till 10cm2.c) Small skin abscesses. B. Major local complications: a) Deep skin necrosis with the area more than 10 cm2. b) Phlegmons. c) fasciitis. The algorithm of treatment of DFE: A) In minor local complications: The opening of pustuled and little abscesses, the excision of limited superficial necroses of skin. B) In major local complications: Aggressive surgical debridement of necrotic outbreak; the daily sanation of the wound after general anesthesia; the treatment of wound with betadin, and hydrosoluble unguents; Vacuum aspiration of the wound; The application of secondary sutures; The application of Ilisarov apparatus in wounds circulars; Autodermoplasty; Drug treatment: antibacterial, infusion therapy, desensitizing, disaggregating, vitamin therapy, etc; Fisioterapeutic therapy. Conclusions: The elaborated classification of local complications of DFE and complex algorithm of treatment can be used by the specialists in everyday practice.
