Introducere. Primele date despre importanța vascularizației grefei osoase transplantate apar în 1905
(Huntington T.W.). Aceasta, după cum menționează autorii, ajută formarea calusului în termeni obișnuiți. În
1975 se întâlnesc primele date despre transfer (liber microchirurgical) reușit de fibulă umană vascularizată
(Taylor G.I.), doi ani mai târziu, aceiași autori descriu prima migrare proximală a fibulei în contextul restituirii
unui defect al femurului distal.
Scopul. Prezentarea unui caz rezolvat de plastie a defectului osos, apărut în urma ablației protezei totale de
genunchi complicată septic, la un pacient oncologic.
Material și metode. Lucrarea prezintă cazul clinic a unei femei de 30 ani, care a fost diagnosticată în 2009 cu
Osteoclastom 1/3 distală a osului femural, operată în același an în Institutului Oncologic,a suportat înlăturarea
tumorii și endoprotezarea totală a articulației genunchiului drept, la sfârșitul anului 2015 se adresează la clinica
noastră cu un focar septic masiv la nivelul membrului pelvin drept. După efectuarea planning-ului preoperator,
am hotărât rezolvarea cazului in 2 etape chirurgicale. La prima etapa am efectuat sanarea focarului și ablația
protezei. În altă etapă am efectuat artrodeza articulației genunchiului drept cu un lambou fibular vascularizat.
Transplantul osos cu o lungime de 20 cm pe un pedicul de 10 cm a fost migrat prin tehnica de răsturnare, în
componența lui a intrat un manșon muscular și o insulă tegumentară. În final membrul inferior a fost stabilizat
intr-un aparat extern extrafocar. După 4 luni distanță, la o vizită repetată, pacienta se deplasează de sine stătător,
cu ajutorul cârjei și sprijin moderat pe piciorul drept.
Concluzii. Utilizarea unui lambou compozit osteo-musculo-cutan vascularizat axial permite reconstrucția
unui defect complicat septic la nivelul membrului pelvin, fără a respecta perioada de remisie necesară.
Introduction. The first data about the importance of the vascularization of bone graft transplant appear in 1905
(Huntington T.W.). As the authors mentioned, this helps callus formation in ordinary terms. In 1975 found the
first data about successful transfer human vascularized fibula (Taylor G.I.) two years later, the same authors
describe the first migration of the proximal fibula, for the replacement of the distal femoral defect.
Purpose. Presenting a solved case of bone defect, that occurred after ablation of the total knee prosthesis
complicated septic in an oncology patient.
Materials and methods. This work presents the clinical case of a woman of 30 years, who was diagnosed in 2009
with osteoclastoma in 1/3 of the distal femur operated in the same year in the Oncology Institute, was removed
the tumor and was done total knee joint prosthesation, at the end of 2015 at our clinic addressed with an septic
area at pelvic right limb. After performing preoperative planning, I decided to solve in 2 surgery stage. At the
first stage we performed ablation of the prosthesis. In another step we made the right knee joint arthrodesis with
a vascularized fibular flap. Bone transplantation with a length of 20cm with a pedicle of 10cm, that was migrated
through rollover technique that in his structure entered a muscular sleeve and skin island for future monitoring.
At the final, the leg was stabilized in an external extrafocar device. After 4 months later, at a follow-up visit, the
patient moves independently, using crutches and moderate support on the foot.
Conclusions. Using a composite musculoskeletal cutaneous vascular defect axially allow reconstruction
complicated septic at pelvic limb without following the required period.
