dc.description.abstract Consumul de droguri are un impact major asupra sănătăţii publice. Scopul studiului a constat în identificarea particularităţilor tuberculozei la utilizatorii de droguri. A fost efectuat un studiu de tip selectiv, descriptiv şi retrospectiv, în care au fost incluşi 48 de bolnavi de tuberculoză înregistraţi în perioada 2013–2016. Au predominat bărbaţii (87%) în vârstă aptă de muncă (100%), cu reşedinţa urbană (73%), iar 17% au fost fără loc de trai. Printre cei investigaţi, 89% au fost economic vulnerabili prin statutul de şomer, 83% – neasiguraţi, 20,8% – cu studii primare sau medii incomplete. Factori epidemiologici agravanţi, precum istoricul de detenţie, au fost constataţi la 33%, contactul tuberculos – la 31%, boli asociate – la 55% din pacienţi. Cazurile noi au constituit 48%, iar cei incluşi în re-tratament – 47%. Un număr limitat (2 pacienţi) nu au iniţiat tratamentul. Forme de tuberculoză pulmonară cu risc letal imediat au fost diagnosticate la 29% pacienţi. Pacienţii pozitivi microbiologic au constituit 50%, iar cei drog-rezistenţi – 17%. Totuşi, doar 8% au fost trataţi cu regim adaptat spectrului de rezistenţă medicamentoasă. Succesul terapeutic a constituit 52%, au decedat 20% şi au fost pierduţi din supraveghere 25% din pacienţi. Aşadar, consumul de droguri este asociat cu multipli factori de risc sociali, epidemiologici şi medicali. Narcomania conferă particularităţi agravante tuberculozei şi risc mare pentru deces sau pierdere din supraveghere. Informarea populaţieiţintă despre faptul că diagnosticul şi tratamentul sunt gratuite indiferent de statutul socioeconomic al pacientului va îmbunătăţi controlul tuberculozei la nivel naţional. en_US
dc.description.abstract Drug use has a major impact on public health. The aim of the study was to identify the particularities of tuberculosis in drug users. A selective, descriptive and retrospective study was carried out involving 48 patients with tuberculosis registered in the period 2013-2016. The results: men (87%), young age (100%, with urban residence (73%), among them 17% were homeless. Economically vulnerable assessed through the status of the unemployed person were 89% patients, lack of health insurance had 83% and primary or incomplete secondary education had 20,8% patients. Severe epidemiological risk factors, such as life history of detention were found in 33%, tuberculosis contact had 31% and co morbid state was established in 55% patients. New cases were 48% and those previously treated were 47%. A limited number (2 patients) did not initiate the treatment. Pulmonary tuberculosis forms with high lethal risk were diagnosed in 29% patients. Microbiological positive were 50% and drug-resistant were 17% patients. However, only 8% were treated with a regimen adapted to the drug resistance spectrum. The therapeutic success was established only in 52%, died 20% and were lost to follow-up 25% patients. In conclusion, the drug use is associated with multiple social, epidemiological and medical risk factors. Drug use aggravates tuberculosis evolution contributing to a high risk for death and loss to follow-up. Informing the key population that diagnosis and treatment is free of charge regardless their socioeconomic state will improve tuberculosis control at national level.
dc.description.abstract Использование наркотиков оказывает серьезное влияние на общественное здравоохранение. Целью исследования было выявить особенности туберкулеза у потребителей наркотиков. Проведено выборочное, описательное и ретроспективное исследование с участием 48 пациентов с туберкулезом, зарегистрированных в период 2013-2016 г. Мужчины составили 87%, молодого возраста (100%), из городской среды были 73% пациентов, из них 17% были бездомными. Экономически уязвимые лица, такие как безработные, составили 89%, без страхового полиса – 83% и 20,8% пациентов имели начальное или неполное среднее образование. Отягощенные эпидемиологические факторы риска, такие как тюремное заключение, было обнаружено у 33% пациентов, туберкулезный контакт – у 31%, сопутствующие заболевание были установлены у 55% пациентов. Новые случаи составили 48%, и повторные случаи – 47%. Два пациента не начали лечение. Легочные формы туберкулеза с высоким летальным риском были диагностированы у 29% пациентов. 50% пациентов были положительными микробиологически, а лекарственно-резистентными были 17% пациентов. Только у 8% пациентов лечение было адаптировано к спектру лекарственной устойчивости. Терапевтический успех был установлен только у 52% пациентов, умерли 20%, а 25% были потеряны из наблюдения. Таким образом, употребление наркотиков связано с многочисленными социальными, эпидемиологическими и медицинскими факторами риска. Наркомания усугубляет тяжесть туберкулеза, что способствует высокому риску смертности и потери из наблюдения. Информирование ключевого населения о том, что диагностика и лечение бесплатны независимо от социально-экономическое статуса улучшит контроль туберкулеза на национальном уровне.
