Introducere: Hemoragia obscură este responsabilă pentru 2-10% dintre cazurile de hemoragie digestivă. Angiodisplazia
mucoasei tractului digestiv este cea mai frecventă cauză, cu o pondere mai mare la persoanele trecute de 50-60 ani.
Caz clinic: Se raportează un caz al unei paciente de 73 de ani, internată repetat în Clinică cu semne de hemoragie digestivă
manifestată prin melenă și anemie. Timp de 2 ani pacienta se spitalizează de 6 ori. Standardul endoscopic superior și inferior
repetat la fiecare spitalizare, irigoscopia, tomografia computerizată cu contrastare nu au relevat sursa de sângerare pe tractul
digestiv. Anemia persistentă și semnele de sângerare continue au impus intervenția chirurgicală. Revizia intraoperatorie a
depistat sânge în ansele intestinului subțire, iar revizia întregului tract digestiv nu a relevat careva patologie organică. A urmat
enteroscopia endoscopică intraoperatorie în sens aferent și eferent prin enterotomie, care a depistat prezența unor mici leziuni
submucoase cu ulcerație centrală și hemoragie activă pe distanță de 30 cm pe ileon; urmată de o rezecție segmentară de ileon
și anastomoză termino-terminală la 50 cm de la unghiul ileocecal. Morfopatologic s-a constatat angiodisplazie de ileon. După o
perioadă de recuperare postoperatorie pacienta a fost externată în stare satisfăcătoare fără semne de hemoragie repetată și cu
ameliorarea evidentă a indicilor hemogramei.
Concluzii: Prezentăm acest caz de hemoragie digestivă recidivantă rară pentru a sublinia rolul enteroscopiei endoscopice în
evaluarea intraoperatorie a tractului digestiv ca procedeu diagnostic de rezervă.
Introduction: Obscure bleeding is responsible for 2–10% of digestive bleeding cases. Digestive tract mucosal angiodysplasia is
the most common cause, with a higher incidence in people over 50-60 years of age.
Clinical case: We report a case of a 73 years old female patient admitted repeatedly with clinical signs of gastrointestinal
bleeding, manifested by anemia and melena. During last 2 years the patient was hospitalized for 6 times. The standard upper
and lower endoscopy, barium enema and contrast enhanced computed tomography performed during every hospitalization,
revealed no source of bleeding in the digestive tract. Persistent anemia and signs of continuing bleeding imposed surgery.
Intraoperatively blood within bowel loops was found and the revision of the digestive tract didn`t reveal any organic pathology.
Intraoperative endoscopic enteroscopy in efferent and afferent loops via enterotomy was performed, which found the presence
of small submucosal lesions with central ulceration and active hemorrhage extending on 30 cm in ileum and segmental ileum
resection with end-to-end anastomosis, at 50 cm from the ileocecal angle. Histopathology showed ileum angiodysplasia. The
postoperative recovery was uneventful and the patient was discharged with no signs of repeated bleeding, but obvious
improvement of the blood count.
Conclusion: We present this rare case of recurrent gastrointestinal bleeding to emphasize the role of endoscopic enteroscopy
for intraoperative evaluation of digestive tract as a backup diagnostic procedure.
