Lansarea transplantului hepatic în Republica Moldova

Abstract

Rezumat: Datele statistice ale OMS în ultimii 10 ani au prezentat o situaţie alarmantă a bolilor cronice difuze hepatice în Republica Moldova, în acelaşi timp ţara noastră este numită ca o zonă cu epidemie a hepatitelor virale HVB şi HCV. Prevalenţa Bolilor Cronice Difuze a Ficatului în 2010 – 2254,07 la 100.000 de populaţie, iar incidenţa constituie 264,39 la 100.000. Rata mortalităţii din cauza cirozei hepatice constituie 76,46 la 100.000 de populaţie şi este pe primul loc în lume. Lansarea transplantului de ficat a devenit necesară urgent pentru a micşora rata mortalităţii la această categorie de bolnavi. Primul pas a fost efectuat în 2008, cînd a fost adoptată Legea privind transplantul de organe. În 2010 a fost deschisă Agenţia de transplant. Primul bolnav cu ciroză hepatică a fost inclus pe lista de aşteptare în 2012. În acelaşi timp a fost nevoie de medici specialişti (chirurgi, anesteziologi, reanimatologi, gastroenterologi, imagişti), care de mulţi ani au realizat stagieri în diferite clinici de transplant din Europa, Asia şi SUA. Deficitul critic de organe disponibile de la donatori aflaţi în moarte cerebrală ne -a impus să utilizăm transplant hepatic de la donator viu, anume cu această metodă am început transplantul hepatic în Republica Moldova. Primul transplant hepatic în Republica Moldova s-a efectuat pe 22 februarie 2013, această operaţie s-a încheat cu succes pentru donator şi recipientă. Primul transplant hepatic de la donator aflat în moarte cerebrală s-a efectuat peste un an, la fel s-a finisat cu succes pentru recipient. La momentul actual în ţara noastră s-au efectuat 14 transplante de ficat, dintre care 7 transplante de la donatori vii şi 7 transplante de la donatori aflaţi în moarte cerebrală. Numărul necesar de 40 transplante de ficat pe an, va fi atins prin implementarea programului naţional de transplant în Republica Moldova.

Summary: Statistical data presented by OMS in the last 10 years have showed a terrible situation in chronic diffuse hepatic diseases in republic of Moldova. In the same time our country is called the epidemic zone of viral hepatitis HBV and HCV. The prevalence of chronic diffuse hepatic diseases – in 2010 was 2254.7 per 100000 of population and incidence was 264.39 per 100000 of population. Mortality rate due to liver cirrhosis is 76.46 per 100000 of population what is on the first place in the world. Launch of liver transplantation become urgent mandatory for decreasing of the mortality in this category of patients. The first step was performed in 2008 when the law of organs transplantation was adopted. In 2010 the Agency of Transplant was founded. The first patient with liver cirrhosis had been introduced in the waiting list in 2012. At the same time it was necessary to prepare the specialists – surgeons, anesthesiologists, reanimatologists, gastroenterologists and others, with experience in different clinics of transplantation in Europe, Asia and USA. The critical insufficiency of available organs from death brain donors required us to use living donor liver transplantation, especially with this method we have begun liver transplantation in Republic of Moldova. The first liver transplantation in Republic of Moldova was performed on February 22nd, 2013. This surgical intervention was successful for the donor and recipient. The first liver transplant from the death brain donor was performed after one year, and it was successful for the recipient. Up to now, 14 liver transplantations have been performed in our country, 7 of them from living donors and 7 from death brain donors. Necessary number of 40 liver transplants every year will be reached due to implementation of national program of transplant in Republic of Moldova.
