Metoda de ligaturare transanală a arterelor hemoroidale cu utilizarea doplerometriei ultrasonore și mucopexie (HAL-RAR) în tratamentul bolii hemoroidale

Abstract

Introducere: Boala hemoroidală ocupă primul loc după răspîndire în afecțiunile coloproctologice. Metodele tradiționale de tratament chirurgical nu permit reîntoarcerea rapidă a pacienților în cîmpul de muncă. Pe lîngă faptul că aceste metode sunt în majoritatea cazurilor mai radicale decît cele miniminvazive există unele momente negative: sindromul dolor pronunțat, hemoragie, dizurie, strictura și insuficiența canalului anal. Material și metode: În 1995 chirurgul japonez Morinaga a publicat primul articol despre o metodă nouă de tratament a hemoroizilor – ligaturarea transanală a ramurilor arterei hemoroidale superioare (HAL). Abordarea nouă a fost băzată pe teoria vasculară a bolii hemoroidale: disfuncție vasculară în regiunea ano-rectală cauzată de creșterea fluxului de sînge arterial și reducerea fluxului din venele cavernoase. De asemenea, un rol important în patogenia bolii hemoroidale îl dețin procesele degenerative în mușchii stratului submucos și ligamentul Parks, care susțin plexul cavernos al canalului anal. Ulterior metoda a fost perfecționată prin RAR (rectoanal repair) – mucopexie, pentru stadiile avansate ale hemoroizilor interni. Rezultate: Aparatul original este prezentat de proctoscop cu transductor ultrasonor Doppler, care prin mîner cu cablu se conecteză la monitor. Proctoscopul cu transductor ultrasonor Doppler transmite semnale către aparat, care le transformă în semnal sonor și o reprezentare grafică pe ecran, ce corespunde pulsației ramurilor arterei hemoroidale superioare și se efectuează ligaturarea arterelor, iar la necesitate “lifting” cu mucopexia mucoasei anale. Concluzii: HAL-RAR este o metodă miniminvazivă eficientă de tratament patogenetic a hemoroizilor.

Introduction: Haemorrhoids have the highest prevalence among coloproctological diseases. Traditional methods of surgical treatment do not allow patients to return to their work quickly. In addition to the fact that these methods are usually more radical than minimally invasive ones, there are some negative aspects, such as: severe pain, bleeding, dysuria, stricture or anal incontinence. Material and methods: In 1995 a Japanese surgeon Morinaga published an article on a new method of treatment of hemorrhoids – transanal ligation of the superior hemorrhoidal artery branches (HAL). The new approach to the treatment was based on the vascular theory of hemorrhoidal disease: vascular dysfunction in the ano-rectal area caused by increase in the inflow of arterial blood and the reduction of the outflow from the cavernous veins. Degenerative processes in the submucosal muscles and Parks ligament, which support the cavernous plexus in the anal canal, also play a key role in the pathogenesis of hemorrhoidal disease. Subsequently, the method has been improved through the RAR (rectoanal repair) – mucopexy for higher grades of internal hemorrhoids. Results: The original unit is represented by the proctoscope with an ultrasonic transductor Doppler, which is connected to the device with the monitor through the handle with the cable. The proctoscope with the ultrasonic transductor transmits signals to the unit, which converts them into the sound and screen graphics that correspond to the pulsation of superior hemorrhoidal artery branches and arteries ligation is done. If it is necessary – lifting with mucopexy of the anal mucosa is performed. Conclusions: HAL-RAR is an effective minimally invasive method in pathogenic treatment of hemorrhoids.
