Introducere: Tromboza venelor profunde (TVP) ale extremităţilor inferioare comportă un risc neprognozabil de trombembolism
pulmonar, inclusiv fatal. Se consideră, că riscul emboligen este maximal în cazurile de TVP flotantă. În localizarea proximală a
trombului flotant sau la bolnavii cu trombembolism pulmonar simptomatic necesită a fi evaluată posibilitatea instalării cavafiltrului,
ca măsură profilactică. Tactica la bolnavii cu TVP flotantă la nivelul segmentului femuro-popliteu nu este însă bine
definită.
Caz clinic: Noi prezentăm cazul clinic al unui bolnav – bărbat, 35 ani, spitalizat iniţial cu trombembolism pulmonar simptomatic
la care s-a diagnosticat tromboza flotantă la nivelul ambelor vene femurale comune. Iniţial, pacientul a suportat un traumatism,
suferind fractura osului tibial drept şi a femurului stîng. Bolnavul este supus intervenţiilor chirurgicale ortopedice la nivelul
ambelor extremităţi inferioare, cu ulterioara imobilizare. Este externat în stare satisfăcătoare cu recomandări de a urma
heparine fracţionate în doză profilactică. Peste 2 săptămîni bolnavul dezvoltă clinică de pneumonie bilaterală şi pleurezie
masivă pe stînga, cu semne radiologice suspecte de trombembolism pulmonar. Examenul ultrasonografic Doppler-duplex a
relevat prezenţa concomitentă a TVP flotante la nivelul ambelor vene femurale comune (apex trombotic nefixat de pereţii venei,
cu mişcări oscilatorii spontane). Se intervine chirurgical, efectuîndu-se trombectomie din vena femurală comună dreaptă, iar
apoi şi cea stîngă, cu ulterioara divizare a lumenului prin plicaţie proximală. Au fost extrase apexurile trombotice libere cu
lungimea de 6,4 cm şi 5,3 cm, respectiv. Perioada postoperatorie a decurs fără particularităţi, pacientul fiind externat la a 8-a zi
postoperatorie.
Concluzii: Cazul clinic descris confirmă posibilitatea dezvoltării TVP în pofida tromboprofilaxiei farmacologice, precum şi
raţionalitatea examinării instrumentale a sistemului venos al extremităţilor inferioare la bolnavii cu trombembolism pulmonar . În
indisponibilitatea tehnicilor intervenţionale endovenoase tratamentul chirurgical poate fi considerat o alternativă eficientă de
profilaxie a trombembolismului pulmonar repetat, cu potenţial fatal, la bolnavii cu TVP flotantă.
Introduction: Deep vein thrombosis (DVT) of the lower extremities possesses an unpredictable, sometimes even fatal, risk of
pulmonary thromboembolism. It is considered, that embolic risk is higher in cases of free-floating DVT. In patients with proximal localization of free-floating thrombus or in those with symptomatic pulmonary embolism it is necessary to evaluate the possibility
of cava-filter placement as a prophylactic measure of recurrent embolism. However, appropriate tactics in patients with freefloating
DVT in femoropopliteal segment is not well defined.
Clinical case: We present the clinical case of a 35-years old male patient hospitalized with symptomatic pulmonary embolism,
which was diagnosed with free-floating DVT in both common femoral veins. Initially, the patient suffered fractures of the right
femur and left tibia, undergoing orthopedic lower limb surgeries, with subsequent leg immobilization. It is discharged in
satisfactory condition with recommendations to follow prophylactic dose of lower molecular weight heparin. Over 2 week patient
develops bilateral pneumonia and massive left pleurisy with radiological signs of pulmonary embolism. Doppler-duplex
ultrasound revealed concomitant free-floating DVT in both common femoral veins (spontaneous oscillatory motion of thrombotic
tip). Patient was submitted for thrombectomy from both, right and then left common femoral vein, with subsequent extra-luminal
interruption by plication. Free-floating parts of thrombi with a length of 6.4 cm and 5.3 cm, respectively, have been extracted.
Postoperative period was uneventful, patient being discharged from the hospital at 8-th postoperative day.
Conclusions: Described clinical case confirms the possibility of DVT occurrence, despite of pharmacological
thromboprophylaxis, and underline rationality of ultrasound exam of venous system of the lower extremities in patients with
pulmonary embolism. In case of unavailability of endovenous techniques surgical treatment can be considered as an effective
alternative for prevention of potentially fatal pulmonary embolism in patients with free-floating DVT.
