Interacţiunile farmacodinamice ale fitopreparatelor şi medicamentelor

Abstract

Interacţiunile farmacodinamice dintre fitopreparate şi medicamente se pot solda cu efecte sinergice, antagoniste şi/sau aditive cu majorarea, diminuarea şi/sau inversarea efectelor terapeutice, cu corecţia şi/sau apariţia reacţiilor adverse, iar in unele cazuri – cu fenomene de intoxicaţie. Mecanismele care stau la baza interacțiunilor dintre medicamente şi fitopreparаte pot fi cauzate de: acţiunea asupra unor ţinte comune sau diverse; reglarea enzimelor implicate in metabolismul hepatic și cel intestinal și transportorilor, cu influenţarea biodisponibilităţii medicamentelor; modificarea afinităţii medicamentelor faţă de receptori; modularea neurotransmisiei; depășirea mecanismelor de rezistență la medicamente ale celulelor microbiene și canceroase; alterarea verigilor patogenetice ale efectelor adverse ale medicamentelor.

The pharmacodynamic interactions between herbal products and drugs may result in synergistic, antagonistic and/or additive effects with increasing, diminishing and/or reversing therapeutic effects, correction and/or occurrence of adverse reactions and, in some cases, with intoxication phenomena. Mechanisms that underlie interactions between drugs and herbal products may be caused by: action on common or various targets; regulation of enzymes involved in hepatic and intestinal metabolism, and transporters influencing the bioavailability of drugs; the modification of the affinity of drugs for receptors; the modulation of neurotransmission; the prevention of drug resistance mechanisms of microbial and cancer cells; the alteration of the pathogenetic links of adverse drug effects.

Фармакодинамические взаимодействия между фитопрепаратами и лекарствами могут приводить к синергидному, антагонистическому и/или аддитивному эффектам с увеличением, уменьшением и/или извращением терапевтических эффектов, коррекцией и/или возникновением неблагоприятных реакций, а в некоторых случаях и с явлениями интоксикации. В основу взаимодействия лекарств и фитопрепаратов могут лежать следующие механизмы: действие на общие или различные точки приложения; регуляция активности ферментов, участвующих в метаболизме в печени и кишечнике, и транспортеров, влияющих на биодоступность лекарств; изменение сродства лекарств к рецепторам; модуляция нейротрансмиссии; преодоление механизмов лекарственной устойчивости микробных и раковых клеток.
