Abstract:
Background. Frostbite are public health problems, with a prevalence rate in the general population of
12.9% in case of moderate forms, and 1.1% in case of severe forms. Nowadays, this topic represents a
special interest due to the possibility to approach it from the perspective of reconstructive
surgery. Objective of the study. To study the clinical evolution of patients with frostbite by III-IV
degree, their compliance with the drug therapy, the frequency of amputation of affected segments and
the rate of complications. Material and Methods. The work was performed on a group of 58 patients
with frostbite by III-IV degree. The analyzed characteristics were: affected area, treatment methods,
antibiotic therapy used, complication rate etc. Results. Of the total number of 58 patients, 37.93% of
the cases had frostbite by III-IV degree of the foot, foot and calf in 24% of cases, and the lower third of
the upper limb in 29% of cases. 36.2% of patients presented at hospitalization a pronounced demarcation
line, as a result, they were subjected to emergency amputation. Other 43.1% cases were applied to the
amputation of this segment. Conclusion. Frostbite is a challenge for modern medicine because it is
difficult to treat and as a result has a high rate of disability (81%). Urgently performing of surgical
debridement and amputations increase the chances of a successful prosthesis. Introducere. Degerăturile sunt o problemă de sănătate publică, cu o rata de prevalență la nivelul
polulației generale de 12,9 % pentru formele medii, și 1,1% pentru formele severe. Astăzi tema prezintă
un inters deosebit datorită posibilității de a o aborda și din perspectiva chirurgiei reconstructorii. Scopul
lucrării. Studierea evoluției clinice a pacienților cu degerături de gradul III-IV, complianța acestora la
tratamentul medicamentos, frecvența amputării segmentelor afectate și rata generală a
complicațiilor. Material și Metode. Lucrarea a fost realizată pe un lot de 58 de pacienți cu congelări de
gradul III-IV. Datele analizate au fost următoarele: zona afectată, tactica de tratament, antibioticoterapia
utilizată, rata complicațiilor etc. Rezultate. Din numărul total de 58 de pacienți, 37,93% au prezentat
degerături de gradul III-IV la nivelul labei piciorului, 24,13% la nivelul labei piciorului și a gambei și
29,31% la nivelul treimii inferioare a membrului superior. 36,2% din pacienți au prezentat în primele
zile de evoluție clinică linia de demarcație pronunțată, prin urmare au fost supuși amputației de urgență.
Alte 43,1% au fost supuși amputației segmentului respectiv în perioada tardivă. Concluzii. Degerăturile
reprezintă o provocare pentru medicina modernă, luând în considerație tratamentul dificil și rata înaltă
de invalidizare (81%). Realizarea necrectomiilor și amputațiilor în regim urgent, cresc șansele unei
ulterioare protezări de succes a membrului afectat.
