Abstract:
Background. Percutaneous nephrolithotomy (PCNL) is the preferred, minimally-invasive, treatment of choice for large (>2 cm) renal or staghorn renal stones. It facilitates a direct approach to the calculus while the kidney is subjected to lesser trauma as compared to the open procedure. Objective of the study. Analysis of the results obtained in the clinic following PCNL intervention applied to patients with urolithiasis, during one year. Material and Methods. The cross-sectional study was performed in the Department of Urology and Surgical Nephrology of the State University of Medicine and Pharmacy "Nicolae Testemitanu", within the Republican Clinical Hospital "Timofei Moşneaga", on a batch of 43
patients with Urolithiasis, treated by the PCNL, during the year 2019. Results. Anatomical distribution of renal stones: right kidney 21 (48,9%) patients, left kidney 22 (51,1%) patients, dimensions ranged between 2 cm and >4,5cm. Postoperative period: 5 days. Stone locations: Renal pelvis 18 (41,8%) patients, caliceal 16 (37,2%) patients, staghorn 9 (21%) patients. Stones dimensions: 2-2,5 cm - 18 (41,86%) patients; 2,6-4 cm - 16 (37,2%) patients; >4,5cm – 9 (20,93%) patients. Postoperative complications by the Clavien-Dindo score: CDS I, 31 (72,1 %) patients; CDS II, 5 (11.6 %) patients;
CDS III, 3 (7 %) patients. CDS IV and V patients were not detected. From the group of patients, 3 (7%) were tubeless and 1 (2,3%) with double-punctured channels. Conclusion. Percutaneous nephrolithotomy success rate depended on: stone composition, size, number, location in the urinary tract, BMI, as well as kidney collecting system anatomy. Overall stone free success rate was 90% and 90-100% after a “second look” procedure, with minimal complications.
Introducere. Nefrolitotomia percutanată (NLP) este tratamentul minim-invaziv, de elecție, pentru calculii renali mari (>2 cm) sau coraliformi. Aceasta facilitează un abord direct asupra calculului, minimalizând trauma renală în comparație cu tactica chirurgicală clasică. Scopul lucrării. Analiza rezultatelor tratamentului pacienților care au suportat NLP. Material și Metode. Studiul transversal a fost realizat în Spitalul Clinic Rebublican „Timofei Moșneaga”, catedra de Urologie si Nefrologie chirurgicală, din cadrul Universității de Stat de Medicină și Farmacie „Nicolae Testemițanu”, pe un lot de 43 de pacienți cu urolitiază, supuși tratamentului prin NLP, pe tot parcursul anului 2019. Rezultate. Distribuția anatomică a nefrolitiazei: rinichiul drept – 21 (48,9%) pacienți, rinichiul stâng – 22 (51,1%) pacienți, dimensiunea calculului a variat între 2 și 4,5 cm. Perioada post-operatorie: 5 zile. Localizarea calculilor: bazinet – 18 (41,8%) pacienți, caliceal –16 (37,2%) pacienți, coraliformi – 9 (21%) pacienți. Dimensiunea calculilor: 2-2,5 cm – 18 (41,86%) pacienți; 2,6-4 cm –16 (37,2%) pacienți; >4,5cm – 9 (20,93%) pacienți. Complicațiile post-operatorii după Clavien-Dindo: CDS I, 31 (72,1 %) pacienți; CDS II, 5 (11.6 %) pacienți; CDS III, 3 (7 %) pacienți. Pacienți cu CDS IV și V nu au fost detectați. Din lotul de pacienți 3 (7%) „Tubeless”, 1 (2,3%) pacienți cu canal dublu punctat. Concluzii. Succesul nefrolitotomiei percutanate a depins de următorii factori: compoziția, dimensiunea, localizarea calculului, IMC, anatomia sistemului calice-bazinetal. S-a înregistrat o medie a ratei „Stone-free” de 90% și în cazul procedurii repetate, de până la 100%, complicațiile fiind minime.
