Clinical importance of thyroid nodules

Abstract

Background. Thyroid nodules, although they are found in the general population with a rate of 50-65%, have become an interdisciplinary clinical problem in the endocrine surgery, unsolved so far due to their malignancy and lack of consensus on diagnostic-curative management. Objective of the study. Optimization of tactics for diagnosis and surgical treatment of thyroid nodules. Material and Methods. A serial clinical study was performed, which included 84 patients (19-69 years), who presented clinically and paraclinically thyroid nodules larger than 1 cm, with rapid growth over a year, without positive dynamics after conservative treatment. In addition, patients were examined by serological tests, Doppler ultrasonography, sonoelastography, scintigraphy, fine needle aspiration - biopsy. Results. Paraclinical benchmarks of thyroid nodules - elevated serum TSH levels, calcitonin, anti-thyroid antibodies; ultrasonography classes TI-RADS 4,5, scintigraphic "cold nodules", cytopathological Bethesda categories IV, V, VI, correlated with the clinical ones determined the surgical treatment. The volume of surgical interventions was established following the result of the extemporaneous histological investigation: hemithyroidectomies - 68 (57%) in benign tumors; total /subtotal thyroidectomy 16 (13%) in malignant tumors. Conclusion. The clinical importance of thyroid nodules consists of the need to assess the malignant potential, dysfunction of the thyroid gland, the degree of compression on adjacent structures, and solving them by selecting the reasonable method of treatment. Introducere. Nodulii tiroidieni, deși se regăsesc în populația generală cu o rată de 50-65%, au devenit în chirurgia endocrină o problemă clinică interdisciplinară, nesoluționată până în prezent datorită caracterului malign și lipsei unui consens asupra managementului diagnostico-curativ. Scopul lucrării. Optimizarea tacticii de diagnostic și tratament chirurgical a nodulilor tiroidieni. Material și Metode. A fost efectuat un studiu clinic seriat, care a inclus 84 pacienți (19-69 ani), ce prezentau clinic și paraclinic noduli tiroidieni mai mari de 1 cm, cu creștere rapidă pe parcursul unui an, fără dinamică pozitivă după tratament conservativ. Suplimentar, pacienții au fost examinați prin teste serologice, ultrasonografia Doppler, sonoelastografia, scintigrafia, puncția-aspirație cu ac fin. Rezultate. Reperele paraclinice ale nodulilor tiroidieni - niveluri crescute ale TSH seric, calcitoninei, anticorpilor antitiroidieni; ultrasonografic clasele TI-RADS 4,5, scintigrafic „noduli reciˮ, citopatologic categoriile Bethesda IV, V, VI, corelate cu cele clinice au determinat tratamentul chirurgical. Volumul intervențiilor chirurgicale s-a stabilit în urma rezultatului investigației histologice extemporanee: hemitiroidectomii - 68 (57%) în tumorile benigne; tiroidectomii totale/subtotale 16 (13%) în tumorile maligne. Concluzii. Relevanța clinică a nodulilor tiroidieni constă în necesitatea aprecierii potențialului malign, dereglării funcției glandei tiroide, gradului de compresie pe structurile adiacente și rezolvarea acestora prin selectarea metodei rezonabile de tratament.
