Periprosthetic femoral fracture in hip arthroplasty revision

Abstract

Background. The incidence rate of performing the hip arthroplasty revision increases because of the extension of the group of patients for whom primary arthroplasty is used. Periprosthetic femoral fractures occur in approximatively from 0,1% to 6% of the patients who supported primary arthroplasty. Objective of the study. The author’s aim was to emphasize the main problems that can occur in prosthetic revision of the hip and also to correlate the data obtained at the Orthopedics and Traumatology Hospital with the international statistics for a period of 3 years. Material and Methods. During the study there were evaluated the results of treatment of patients with prosthetic revision of the hip for a period of 3 years from the clinics. As materials there have been used the clinical observation sheets, outpatient observation sheets, surgery protocols and radiological images. Results. There were reported a number of patients that needed a prosthetic revision. Thus, out of the total number of 790 primary prostheses for a period of 3 years, 146 patients needed an arthroplasty revision of the hip. Thereby, 23 patients were diagnosed with periprosthetic femoral fracture, 61% females and 39% males, and the prevalent age was 70-80 years. Conclusion. The author demonstrated that the incidence rate of periprosthetic femoral fracture is 3% which is included in international limits, where it reaches up to 6% and is mainly found in female ortho-geriatric patients suffering from osteopenia. Introducere. Rata de incidență de efectuare a intervențiilor de revizie a articulației șoldului crește în urma extinderii arealului de pacienți pentru care se utilizează artroplastia primară. Fracturile periprotetice apar în aproximativ 0,1% - 6% la pacienții ce au beneficiat de artroplastie primară. Scopul lucrării. Autorul articolului a avut drept scop de a pune în discuție principalele probleme pe care le pune artroplastia de revizie și de a corela datele preluate din Spitalul Clinic de Traumatologie și Ortopedie pe o perioadă de 3 ani în corelație cu datele internaționale. Material și Metode. Pe parcursul studiului au fost evaluate rezultatele tratamentului bolnavilor cu artroplastie de revizie a șoldului pe o perioadă de 3 ani din cadrul clinicii. S-a folosit în calitate de materiale: fișe de observație clinică, fișe de ambulatoriu, protocoale ale operațiilor și imagini radiologice. Rezultate. Sunt raportate o serie de pacienți care aveau nevoie de intervenție de revizie. Astfel, din totalul de 790 de protezări primare timp de 3 ani, 146 de pacienți au beneficiat de endoprotezare de reluare, dintre care 23 de pacienți manifestau fracturi periprotetice. Dintre aceștia, 61% erau femei și 39% barbați, iar vârsta preponderentă era 70-80 ani. Concluzii. În urma studiului, s-a demonstrat că frecvența fracturilor periprotetice este de 3%, încadrându-se în limitele internaționale, unde aceasta ajunge până la 6% și se întâlnește preponderent la pacienții orto-geriatrici de gen feminin ce suferă de osteopenie.
