Abstract:
Background. MELD score and derived MELD scores estimate survival at 3 months in patients with
decompensated liver cirrhosis on the waiting list for liver transplantation. It is unclear whether these
scores would be important in the prognosis of short-term mortality after liver transplantation. Objective
of the study. Assessment of the ability of MELD score and derived MELD scores to predict short-term
post-transplant mortality in patients who received liver transplantation in the Republic of
Moldova. Material and Methods. A retrospective cohort study was performed, analyzing data from 53
patients who received a liver transplant in 2013-2020. The MELD score and its modified variants
(MELD-Na, iMELD, uMELD, UKELD) were used to test the predictive power of short-term posttransplant mortality, compared by ROC analysis and diagnostic accuracy. Results. The final group
consisted of 53 patients with a mean age of 47.4 years. The most common indication for transplantation
was cirrhosis due to HDV (49%). The mortality rate at 3 months after transplantation was 17%. The
MELD score obtained an area under the ROC curve - 0.68, and from the derived MELD scores the
greatest areas were reported at iMELD - 0.77, followed by UKELD - 0.72. MELD-Na stood out with
the highest diagnostic accuracy - 79.25% and specificity - 81.4%. The highest sensitivity was reported
at iMELD - 88.9%, but also the lowest specificity - 52.3%. Conclusion. The MELD score did not prove
to be a useful tool in predicting short-term post-transplant mortality, and of the derived MELD scores
the most effective in this regard was MELD-Na.
Introducere. Scorul MELD și scorurile MELD derivate estimează supraviețuirea la 3 luni la pacienții
cu ciroză hepatică decompensată ce se află în lista de așteptare pentru transplant hepatic. Nu este clar
dacă aceste scoruri ar avea importanță în prognosticul mortalității pe termen scurt după transplant
hepatic. Scopul lucrării. Evaluarea capacității scorului MELD și a scorurilor MELD derivate de a
prezice mortalitatea pe termen scurt posttransplant la pacienții ce au beneficiat de transplant hepatic în
Republica Moldova. Material și Metode. A fost realizat un studiu retrospectiv de cohortă, fiind
analizate datele de la 53 pacienți care au beneficiat de transplant hepatic în 2013-2020. Au fost utilizate
scorul MELD și variantele sale modificate (MELD-Na, iMELD, uMELD, UKELD) pentru a testa
puterea de predicție a mortalității pe termen scurt posttransplant, comparate prin analiza ROC și
acuratețea diagnostică. Rezultate. Lotul final a constituit 53 pacienți cu vârsta medie de 47,4 de ani.
Cea mai frecventă indicație pentru transplant a fost ciroza hepatică de etiologie D (49%). Rata de
mortalitate la 3 luni posttransplant a fost de 17%. Scorul MELD a obținut o arie sub curba ROC – 0,68,
iar din scorurile MELD derivate cele mai mari arii au fost raportate la iMELD – 0,77, fiind urmat de
UKELD – 0,72. MELD-Na s-a evidențiat prin cea mai înaltă acuratețe diagnostică – 79,25% și
specificitate – 81,4%. Cea mai înaltă sensibilitate a fost raportată la iMELD – 88,9%, dar și cea mai
joasă specificitate – 52,3%. Concluzii. Scorul MELD nu s-a dovedit a fi un instrument util în predicția
mortalității pe termen scurt posttransplant, iar din scorurile MELD derivate cel mai eficace în acest sens
a fost MELD-Na.
