Abstract:
Background. Hepatitis E Virus Infection represents a global public health issue, as well as for the
Republic of Moldova. Given this, the European Association for the Study of the Liver created in 2018
a practical guide of clinical conduct, epidemiology, diagnosis, treatment, and prophylaxis. Objective of
the study. Evaluation of clinical-epidemiological and diagnostic features in a familial outbreak of viral
hepatitis E. Material and Methods. The study constituted of 4 members of a young family: mother,
father, and 2 children, aged 4 years and 1 year and 6 months. Results. The family went on a vacation in
Egypt from 16-23.03.19. The first gets sick mother with nausea, vomiting twice, loss of appetite, general
weakness - lasting a week, then the condition improved. Then the 2 children get sick, with the same
clinical symptoms, which improve quickly. The latter ill father with - nausea, vomiting, loss of appetite,
pain in the right costal rim, acholic stool, general malaise. From investigations – hypertransaminazemia
and directed to HCID "Toma Ciorbă", where was confirmed viral hepatitis E. From the epidemiological
investigation - the mother sometimes used tap water and the personal hygiene was not always respected
in the family. Conclusion. A family outbreak has been reported with hepatitis E, in which the first
contact was most likely, via water. The disease in the first three patients evolved into a mild form and
in latter case evolved into a medium form, which was confirmed clinically and by long term
transaminesmia. Introducere. Hepatita virală E prezintă o problemă de sănătate publică mondială, inclusiv și pentru RM.
Reieșind din importanța acestei infecții, Asociația Europeană de Studiu al Ficatului în anul 2018 a
elaborat un ghid practic de conduită clinică, epidemiologică, de diagnostic, tratament și
profilaxie. Scopul lucrării. Evaluarea particularităților clinico-epidemiologice și de diagnostic într-un
focar familiar de hepatită virală E. Material și Metode. În studiu s-au aflat 4 pacienți, membrii unei
familii tinere: mama, tata și 2 copii, cu vârsta de 4 ani și 1 an și 6 luni. Rezultate. O familie cu 2 copii
s-a aflat la odihnă în Egipt în perioada 16-23.03.19. Prima se îmbolnavește mama, acuzând greață, vomă
de 2 ori, inapetență, slăbiciune generală – cu o durată de o săptămână, apoi starea s-a ameliorat. Apoi,
se îmbolnăvesc cei 2 copii, cu aceleasi simptome clinice, ce se ameliorează rapid. Ultimul se
îmbolnăvește tata – greață, vomă, inapetență, dureri în rebordul costal drept, scaun acholic, stare
generală de rău. Din investigații – hipertransaminazemie, fiind direcționat la SCBI „Toma Ciorbă”, unde
este confirmată hepatita virală E. Din ancheta epidemiologică - mama uneori utiliza apă din robinet, în
familie igiena personală nu întotdeauna era respectată. Concluzii. S-a prezentat un focar familial de
hepatită cu virusul E care, în primul caz, a fost transmis probabil pe cale hidrică, apoi habitual. Maladia
la primii trei pacienți a evoluat într-o formă ușoară, iar în ultimul caz a evoluat în formă medie,
confirmată clinic și prin transaminazemie de durată.
