Assessment of the stage of the tumor and the volume of surgery in extrahepatic bile duct and Vater papilla cancer

Abstract

Background. Cancer of the extrahepatic bile ducts and papilla Vater occupies the 20th place (0.4% o) in the structure of oncological morbidity in the Republic of Moldova. Aggressive evolution and late diagnosis are argued by increased mortality, which represents 10.5%o. Objective of the study. To assess the stage of the tumor and the volume of surgery in extrahepatic bile duct and Vater papilla cancer. Material and Methods. The retrospective study includes 38 patients with malignant tumors of the extrahepatic bile ducts and Vater papilla under the Diagnostic Consultative Center of the Oncological Institute during 2016-2019, with a mean age of 62.7. Results. Evaluation of the site of the primary tumor detected proximal bile duct cancer (Klatskin's tumor) in 44.7% of cases; Vater's papilla cancer - in 42.1% and choledochal cancer - in only 13.2%. At the stage of establishing the diagnosis, regardless of location, the advanced stages were predominantly recorded in 30 cases (stage III - 23.7% and stage IV - 55.2%), which underwent only palliative treatment, at the same time stage I and II was assessed in 8 cases (21.1%), in which radical surgical treatment was possible. Conclusion. The late diagnosis in the advanced stages, which constitutes 78.9%, determines in the vast majority of cases the application of palliative surgical treatment, which corresponds to the current statistical data. Introducere. Cancerul căilor biliare extrahepatice și al papilei Vater ocupă locul 20 (0,4%o) în structura morbidității oncologice din Republica Moldova. Evoluția agresivă și diagnosticul tardiv sunt argumentate prin mortalitatea crescută, ce reprezintă 10,5%. Scopul lucrării. Aprecierea stadiului tumorii și volumul operației în cancerul căilor biliare extrahepatice și a papilei Vater. Material și Metode. Studiul retrospectiv include 38 de pacienți cu tumori maligne ale căilor biliare extrahepatice și ale papilei Vater aflați la evidența Centrului Consultativ Diagnostic al Institutului Oncologic în perioada 2016-2019, cu vârsta medie de 62,7. Rezultate. Evaluarea sediului tumorii primare a depistat cancerul căii biliare proximale (Tumoră Klatskin) în 44,7% cazuri; cancerul papilei Vater – în 42,1% și cancerul de coledoc – în doar 13,2%. La etapa stabilirii diagnosticului, indiferent de localizare, s-au înregistrat predominant stadiile avansate în 30 cazuri (stadiul III – 23,7% și stadiul IV - 55,2%), care au suportat doar tratament paliativ, în același timp stadiul I și II a fost apreciat în 8 cazuri (21,1%), în care a fost posibil tratamentul chirurgical radical. Concluzii. Diagnosticul tardiv în stadiile avansate, ce constituie 78,9%, determină în marea majoritate dintre cazuri aplicarea tratamentului chirurgical paliativ, ceea ce corespunde cu datele statistice actuale.
