Diagnostic value of sympathetic skin responses determined by maximum voluntary jaw contraction in patients with autonomous disorders

Abstract

Background. There are various methods for determining the sympathetic skin responses, based on the electrical stimulation of various body regions, these being associated with an emotional anticipation stress, which influences the results of investigations in patients with autonomous suprasegmental disorders. Objective of the study. Comparative determination of the diagnostic value of the sympathetic skin responses in conditions of electrical stimulation and standardized maximum voluntary jaw contraction. Material and Methods. In this prospective analytical study, there were enrolled 45 patients (15 - cerebellar tumors in the postop period, 15 - facial myalgias, 15 - hyperactive bladder). There were determined the indices of Vein, Valkova questionnaires, sympathetic skin responses (Amax, mV) in various test conditions, with the analysis of the sensitivity (Se), specificity (Sp) and efficiency (Ef) of these methods. Results. Patients were diagnosed with autonomous suprasegmental clinical disorders of varying intensity. The Amax range was between 2.0 and 44 mV. The diagnostic value for percutaneous electrical stimulation: Se = 85%, Sp = 78%, Ef = 81.5%; for standardized maximum voluntary jaw contraction with cotton rolls between molars: Se = 94%, Sp = 90%, Ef = 92%. Conclusion. The determination of sympathetic skin responses in standardized conditions has a significant diagnostic value, it is performed in physiological conditions, in the absence of anticipation stress, thus optimizing the process of diagnosis and monitoring.

Există diferite metode de determinare a potențialului cutanat simpatic, bazate pe stimularea electrică a diferitor regiuni corporale, metodele fiind asociate cu un stres emoțional de așteptare, care influențează rezultatele investigațiilor la pacienții cu dereglări autonome suprasegmentare. Scopul lucrării. Determinarea comparativă a valorii diagnostice a potențialului simpatic cutanat în condiții de stimulare electrică și angrenare forțată standardizată a maxilarelor. Material și Metode. În studiul prospectiv analitic au fost înrolați 45 de pacienți (15 – cu tumori cerebelare în perioada posteoperatorie, 15 – mialgii faciale, 15 – vezică hiperactivă). S-au determinat indicii chestionarelor Vein și Valkova, potențialului simpatic cutanat (Amax, mV) în diferite condiții de testare, cu analiza sensibilității (Se), specificității (Sp) și eficienței (Ef) metodelor studiate. Rezultate. Pacienții au prezentat dereglări clinice autonome suprasegmentare de diferită intensitate. Diapazonul Amax a fost între 2,0 și 44 mV. Valoarea diagnostică la stimularea electrică percutanată: Se = 85%, Sp = 78%, Ef = 81.5%; la angrenarea standardizată a maxilarelor cu rulouri între molari: Se = 94%, Sp = 90%, Ef = 92%. Concluzii. Determinarea potențialelor simpatice cutanate în condiții de standardizare are o valoare diagnostică semnificativă, se realizează în condiții fiziologice, în lipsa stresului emoțional de așteptare, astfel se optimizează realizarea procesului diagnostic și de monitorizare.
