Rezumat
Deși în ultimul secol incidența cancerului gastric a scăzut semnificativ, maladia este încă una dintre principalele cauze de
deces prin cancer la nivel mondial. În cele mai recente statistici
globale de cancer, afecțiunea ocupă al 5-lea loc după incidență
(8% din numărul total de cancere) și reprezintă a 2-a cauză
principală de deces prin cancer la ambele sexe. În România,
cancerul gastric este a 6-a localizare malignă la ambele sexe,
cu o incidență de 5,2%. Chiar dacă depistarea la timp și tratamentul timpuriu sunt posibile, majoritatea cazurilor de cancer
gastric sunt diagnosticate în stadiu avansat, ceea ce duce la o
rată de supraviețuire la cinci ani de 20-30%. Din acest motiv,
evidențierea grupelor de risc pentru dezvoltarea cancerului
gastric (pacienții cu leziuni precanceroase – gastrita cronică
atrofică, metaplazia intestinală gastrică, displazia epiteliului
mucoasei gastrice) și supravegherea atentă a acestora este o
prioritate în vederea creșterii depistării timpurii și, implicit,
a scăderii morbidității și a mortalității provocate de această
maladie. Prevalența gastritei cronice atrofice variază de la
0% până la 9,0%, iar în zonele cu incidență ridicată – de la
33% până la 84%. Prevalența metaplaziei intestinale gastrice
variază foarte mult – de la 3,4% până la 29,6%, iar cea a
displaziei epiteliului mucoasei gastrice este de 1,4-5,8%.
Astfel, datele epidemiologice privind prevalența, incidența și
trendurile cu vârsta ale gastritei cronice atrofice, metaplaziei
intestinale gastrice și displaziei epiteliului mucoasei gastrice
sunt neomogene din cauza diferențelor metodologice dintre
studii.
Summary
Although the incidence of gastric cancer has decreased significantly in the last century, the disease is still one of the leading causes of death from cancer worldwide. In the latest global cancer statistics, the disease ranks fifth by incidence (8% of the total number of cancers) and is the second leading cause of cancer death in both sexes. In Romania, gastric cancer is the sixth malignant localization in both sexes, with an incidence of 5,2%. Even if early detection and treatment are possible, most cases of gastric cancer are diagnosed at an advanced stage, resulting in a 5-year survival rate of 20-30%. For this reason, the detection of risk groups for the development of gastric cancer (patients with precancerous lesions – chronic atrophic gastritis, gastric intestinal metaplasia, dysplasia of the epithelium of the gastric mucosa) and their close monitoring is a priority to increase early detection and, consequently, decrease mortality and morbidity of this disease. The prevalence of chronic atrophic gastritis varies from 0% to 9%, and in areas with high incidence – from 33% to 84%. The prevalence of gastric intestinal metaplasia varies greatly – from 3,4% to 29,6%, and the prevalence of gastric mucosal epithelial dysplasia is 1,4-5,8%. Thus, epidemiological data on the prevalence, incidence and age trends of chronic atrophic gastritis, gastric intestinal metaplasia and dysplasia of the epithelium of the gastric mucosa are inhomogeneous due to methodological differences between studies.

Хотя заболеваемость раком желудка значительно снизилась за последнее столетие, эта болезнь попрежнему является одной из основных причин смерти от рака во всем мире. В последней мировой статистике по раку он занимает пятое место по заболеваемости (8% от общего числа раковых заболеваний) и является 2-й по значимости причиной смерти от рака у обоих полов. В Румынии рак желудка является 6-й злокачественной локализацией у обоих полов с частотой 5,2%. Даже если возможно раннее выявление и лечение, большинство случаев рака желудка диагностируется на поздней стадии, в результате 5-летняя выживаемость составляет 20-30%. По этой причине выявление групп риска по развитию рака желудка (пациентов с предраковыми заболеваниями желудка – хронический атрофический гастрит, желудочная метаплазия кишечника, дисплазия эпителия слизистой оболочки желудка) и их тщательный мониторинг является приоритетом для повышения раннего выявления и, следовательно, снижается смертность и заболеваемость. Распространенность хронического атрофического гастрита колеблется от 0% до 9,0%, а в районах с высокой заболеваемостью – от 33% до 84%. Распространенность кишечной метаплазии желудка сильно варьирует – от 3,4% до 29,6%, а распространенность дисплазии эпителия слизистой желудка – 1,4-5,8%. Таким образом, эпидемиологические данные о распространенности, заболеваемости и возрастных тенденциях хронического атрофического гастрита, желудочной метаплазии кишечника и дисплазии эпителия слизистой оболочки желудка неоднородны из-за методологических различий между исследованиями.
