Aprecierea eficacităţii tratamentului intravitrean cu inhibitori anti-VEGF în degenerescența maculară legată de vârstă, forma exsudativă, aplicând tomografia în coerență optică

Abstract

Rezumat Degenerescența maculară legată de vârstă (DMLV), forma exsudativă, este una dintre problemele majore ale oftalmologiei. Metodele moderne de diagnostic și de tratament pot încetini progresarea patologiei, iar cercetările continue pot descoperi noi modalități de stopare și inversare a proceselor degenerative ale retinei. Studiul a fost efectuat în baza rezultatelor examinării a două loturi de pacienți investigați prin tomografia în coerență optică (TCO): lotul de bază (62 de pacienți, 62 de ochi) cu tratament intravitrean cu bevacizumab și lotul-martor (40 de pacienți, 40 de ochi) cu tratament tradițional. La pacienții din lotul de bază, funcțiile vizuale s-au îmbunătățit după prima injecție intravitreană, majorarea acuității vizuale s-a înregistrat în 82% cazuri, micșorarea grosimii maculei – în 85% cazuri determinate prin TCO, în comparație cu lotul de control, în care nu s-au constatat modificări.

Summary Wet form of age-related macular degeneration is an increasing problem in ophthalmology. Current treatment options can delay progression and continue researches can discover new ways to halt and reverse the degenerative processes of the retina. The study was made on the examination results of two groups of patients that were investigated applying optical coherence tomography (OCT): the first basic group (62 patients, 62 eyes) that followed an intravitreal treatment with Bevacizumab and the control group (40 patients, 40 eyes) that followed the usual treatment. The visual functions of the patients from the first basic group were improved after the first intravitreal injection, visual acuity was increased in 82% cases and macula’s thickness in 85% determined by OCT in comparison with the control group which did not track changes.

Резюме Влажная форма возрастной дегенерации макулы является одной из основных проблем в офтальмологии. Современные методы диагностики и лечения могут приостановить прогрессирование патологии, а текущие исследования способны выявить новые способы замедления дегенеративных процессов в сетчатке. Исследование проводилось на основании результатов обследования двух групп пациентов, которые были исследованы с помощью оптической когерентной томографии (ОКТ): основная группа (62 пациента, 62 глаза), получавшие интравитреальное лечение бевацизумабом, и контрольная группа (40 пациентов, 40 глаз), получавшие традиционное лечение. У пациентов из основной группы зрительные функции улучшились после первой интравитреальной инъекции, повышение остроты зрения было установлено в 82% случаев, уменьшение толщины макулы – в 85% случаев, которые были определены с помощью ОКТ, по сравнению с контрольной группой, в которой никаких изменений не наблюдалось.
