Abstract:
Background. ACPA and FR are markers that determine the serological type of rheumatoid arthritis
(RA): seropositive (spRA) and seronegative (snRA). The sensitivity and specificity of the ACR /
EULAR 2010 Criteria is 75%, therefore it is proposed to apply the Ultrasonography (US) in the early
diagnosis of snRA. Objective of the study. Determining the importance of the US examination in the
early diagnosis of snRA and predictive differences depending on seropositivity. Material and
Methods. 102 patients were included, with symptoms lasting up to 2 years. The RC joints, MCP (II-V),
PIP, MTP, TC were examined clinically and US. CRP, ESR, RF, ACPA, Ritchie Articular Index, and
VASs were evaluated. The presence of synovial proliferation, synovial vascularization (with Power
Doppler (PD)), the presence of erosions, tendinitis and tenosynovitis was evaluated by
USG. Results. Patients were divided into 2 groups. Group I - 65 (64%) patients with spRA (RF and / or
ACPA positive) and group II - 37 (36%) patients with snRA (RF and ACPA negative). The mean age
was 66.5 years (33-67) vs. 58.5 years (23-71). The number of painless and swollen joints was
insignificant in the II group. The DAS 28 in group I-4.48(3.42-5.65) vs 5.18(4.31-6.18). SIV - 50 vs 70,
which was significantly higher in group II. At the US examination - the presence of synovitis was
significantly higher in the seronegative group. The PD score ≥ 2 was significantly higher in the
seronegative group. The presence of erosions was in a significantly higher number in
spRA. Conclusion. It was found that in patients with snRA the inflammatory activity is more eloquent
than in those who are seropositive. The results of the ultrasonographic examination found its value in
the correctness of pre-establishing the early diagnosis of RA. Introducere. ACPA și FR sunt marcheri ce determină tipul serologic al artritei reumatoide (AR):
seropozitivă (ARsp) și seronegativă (ARsn). Sensibilitatea și specificitatea criteriilor ACR/EULAR
2010 constituie 75%, de aceea se propune aplicarea examenul ultrasonografic (USG) în diagnosticul
precoce al ARsn. Scopul lucrării. Determinarea importanței examenului USG în diagnosticul precoce
al ARsn și diferențele predictive în funcție de seropozitivitate. Material și Metode. Au fost incluși 102
pacienți. Durata simptomelor a fost de până la 2 ani. Au fost examinate clinic și USG articulațiile RC,
MCF (II-V), IFP, MTF, TC. Au fost evaluate Scorul DAS, PCR, VSH, FR, ACPA, SIV, indicele
articular Ritchie. Cu ajutorul USG au fost evaluate prezența proliferării sinoviale, evaluarea
vascularizării sinoviale cu Power Doppler (PD), prezența eroziunilor, tendinitelor și
tenosinovitelor. Rezultate. Pacienții au fost divizați în 2 grupe. Grupul I – 65 (64%) cu AR sp (FR sau/și
ACPA pozitive) și grupul II – 37 (36%) cu AR sn (FR și ACPA negativ). Vârsta medie a fost 66.5 ani
(33-67) vs 58.5 ani (23-71). Numărul de articulații dolore și tumefiate nesemnificativ a predominat în
grupul II. Scorul DAS 28 - în grupul I-4.48(3.42-5.65) vs 5.18(4.31-6.18). SIV – 50 vs 70, ce a fost
semnificativ mai înalt în grupul II. La examenul USG – prezența sinovitei a fost semnificativ mai înaltă
în grupul seronegativ. Scorul PD ≥ 2 a fost semnificativ mai înalt în grupul seronegativ comparativ cu
cel seropozitiv. Prezența eroziunilor a fost într-un număr semnificativ mai mare în AR sp. Concluzii:
Am constatat că la pacienții cu AR seronegativă activitatea inflamatorie este mai elocventă decât la cei
seropozitivi. Rezultatele examenului ultrasonografic au constatat valoarea acestuia la corectitudinea
prestabilirii diagnosticului precoce a AR.
