Abstract:
Background. Autoimmune hypothyroidism is one of the most common organ-specific autoimmune
disorders. There are several correlations between autoimmune hypothyroidism and subclinical
inflammation, but most studies do not evaluate the change in inflammatory biomarkers during
treatment. Objective of the study. To evaluate the effect of levothyroxine therapy on biomarkers of
subclinical inflammation in patients with hypothyroidism. Material and Methods. The study included
19 patients with recently diagnosed hypothyroidism, who were not treated with hormone replacement
therapy. 11 patients were investigated after 2 and 4 months of treatment with levothyroxine with an
average dose of 0.7-1.6 mcg / kg / day. The investigated parameters were TSH, fT4, hs-CRP, ESR,
fibrinogen. The t-Student statistical test was performed. Results. Out of 19 patients with elevated TSH,
15 had low fT4 (overt hypothyroidism) and 4 had normal fT4 (subclinical hypothyroidism). All the
patients had increased anti- thyroperoxidase and/or anti-thyroglobulin antibodies level. There was a
significant decrease in TSH over time (P<0.0001) (initial levels were on average 56,29 μIU / mL and și
2,33 μIU / mL at 4 months). There was a significant increase in FT4 (P < 0.005) (initial levels were on
average 0,56 ng / dL and și 1,2 ng / dL at 4 months). A decreasing trend of hs-CRP was observed over
time (initial levels were on average 2.29 mg / L, 1.83 mg / L at 2 months and 2.19 mg / L at 4 months,
P = 0,23). Conclusion. This study showed a tendency in decrease of hs-CRP in patients with
hypothyroidism under treatment. However, the small sample size and the heterogeneous group of
patients (both with clinical and subclinical hypothyroidism) impose further research.
Introducere. Hipotiroidia autoimună este una din cele mai frecvente afecţiuni autoimune organspecifice.
Există mai multe corelații între hipotiroidia autoimună şi inflamaţia subclinică, dar majoritatea
studiilor nu evaluează modificarea biomarkerilor inflamatori în timpul tratamentului. Scopul
lucrării. Cercetarea efectului terapiei de substituţie cu levotiroxină asupra biomarkerilor inflamației
subclinice la pacienții cu hipotiroidie. Material și Metode. În studiu au fost incluşi 19 pacienţi cu
hipotiroidie primar depistată, care nu administrau tratament de substituţie. Au fost investigați 11 pacienți
la interval de 2 şi 4 luni de tratament cu levotiroxină în doza de 0,7-1,6 mcg/kg/zi. Parametrii cercetaţi
au fost TSH, fT4, hs-CRP, VSH, fibrinogen. Testele statistice utilizate au fost t-Student. Rezultate. Din
cei 19 pacienţi cu TSH crescut, 15 aveau fT4 scăzut (hipotiroidie manifestă), iar ceilalţi 4 – fT4 normal
(hipotiroidie subclinică). Toți pacienții au avut niveluri crescute de anticorpi antitireoperoxidază și / sau
anti-tireoglobulină. A fost apreciată o scădere semnificativă a TSH în timp (P<0,0001) (nivelurile
inițiale au fost în medie 56,29 μIU / mL și 2,33 μIU / mL la 4 luni). A fost o creștere semnificativă a
FT4 (P<0,005) (nivelurile inițiale au fost în medie de 0,56 ng / dL și 1,2 ng / dL la 4 luni). A fost
observată o tendință de scădere a hs-CRP în timp (nivelurile inițiale au fost în medie 2,29 mg/L, 1,83
mg/L la 2 luni și 2,19 mg/L la 4 luni, P = 0,23). Concluzii. Acest studiu a evidenţiat o tendință de scădere
a hs-CRP la pacienții cu hipotiroidie aflați sub tratament. Totuși, trebuie să constatăm că mărimea mică
a eșantionului și lotul heterogen de pacienţi (atât cu hipotiroidie clinică, cât şi subclinică) impune
continuarea cercetărilor.
