Abstract:
Background. Breast implants are gaining in popularity, being one of the most common surgeries
required by women. Currently, imaging explorations are becoming indispensable for establishing the
complex clinical-imaging diagnosis of the pathology of the mammary glands with implant. The problem
of the imaging management of the mammary glands with implant consists in establishing the imaging
exploration protocol, which would correspond to a rigorous methodology, systematized according to the
last requirements - more effective, less traumatic and as fast as possible. Objective of the study. To
estimate the sensitive and specific value of MRI and USG imaging investigations of breast implant
patients in order to establish the imaging behavior of the implant mammary gland. Materials and
methods: The prospective study included 36 patients with breast implants who underwent ultrasound
examination (USG) of the mammary glands and magnetic resonance imaging (MRI) with contrast
administration. Data were reported according to the international classification BI-RADS (Breast
Imaging Reporting and Data System). The results obtained were compared by the 2 ways that were
performed. Results: The results obtained will be used to develop a customized imaging algorithm for
these patients and diagnose breast implant complications through medical imaging methods, finally
estimating the sensitivity, specificity and effectiveness of each imaging method separately and in
complexity.
Introducere. Implanturile mamare capată o popularitate tot mai înaltă, fiind una din cele mai frecvente
intervenții chirurgicale solicitate de femei. Actualmente, explorările imagistice devin indispensabile
pentru stabilirea diagnosticului clinico-imagistic complex al patologiei glandelor mamare cu implant.
Problema managmentului imagistic al glandelor mamare cu implant constă în stabilirea protocolului de
explorare imagistică, care ar corespunde unei metodologii riguroase, sistematizate după ultimele cerinţe
– mai eficace, mai puţin traumatizantă şi cât mai rapid posibilă. Scopul lucrării. Estimarea valorii
senzitive și specifice a investigaţiilor imagistice prin IRM și USG ale pacientelor cu implant mamar în
vederea stabilirii conduitei imagistice a glandei mamare cu implant. Materiale și metode. Studiul
prospectiv a inclus 36 de paciente cu implant mamar, care au efectuat examenul ultrasonografic (USG)
al glandelor mamare și investigație prin rezonanță magnetică (IRM) cu administrarea substanței de
contrast. Datele au fost raportate conform clasificarii internaționale BI-RADS (Breast Imaging
Reporting and Data System). Rezultatele obținute au fost comparate prin cele 2 modalități care au fost
efectuate. Rezultate. Rezultatele obținute vor servi la elaborarea unui algoritm imagistic personalizat
pentru aceste paciente și diagnosticarea complicațiilor implantului mamar prin intermediul metodelor
imagisticii medicale, estimând, în final, sensibilitatea, specificitatea şi eficacitatea fiecărei metode
imagistice separat şi în complexitate.
