Abstract:
Background. Liver stiffness is a prognostic marker of the evolution of HCV infection. The goal of
antiviral therapy in HCV is to obtain a sustained virological response, but also the regression of liver
fibrosis. Objective of the study. The objective of this study was to evaluate liver stiffness after
treatment with direct-acting antiviral agents in patients with cirrhosis, and to assess the effectiveness of
this therapy in these patients. Material and Methods. The prospective, randomized study included 74
patients with HCV and cirrhosis, who received treatment with direct-acting antiviral agents. Patients
were divided into two groups: group I - sofosbuvir and daclatasvir / ledipasvir with ribavirin for 12
weeks and group II - sofosbuvir and daclatasvir / ledipasvir for 24 weeks. Results. Distribution of
patients: group I - 38 (51.3%) patients, group II - 36 (48.6%). Distribution of patients according to the
stage of fibrosis at the initiation of therapy: stage F3 (12.5 - 13.9 kPa) in 4 (5.4%) patients, stage F4 -
70 (94.5%). At 6 months and 12 months after the end of therapy there was an improvement in liver
fibrosis in both groups: in group I the mean value of liver fibrosis at the initiation of antiviral therapy,
at 6 and 12 months after treatment was 31 ± 14.0 kPa, 25, 3 ± 10.7kPa and 20.3 ± 10.1kPa; in group II
it was 28.5 ± 10.0kPa, 28.2 ± 11.6kPa and 24.3 .1 kPa. Sustained virologic response was - 68 (91.9%)
patients, treatment failure - 6 (8.1%). Conclusion. Treatment regimens containing sofosbuvir and
daclatasvir / ledipasvir with / without ribavirin have shown promising results and are a good treatment
option in cirrhosis with HCV. The results of our study revealed an improvement in liver fibrosis at 6
and 12 months after this therapy.
Introducere. Fibroza hepatică este un marker de prognostic privind evoluţia infecţiei cu VHC. Scopul
terapiei antivirale în VHC este obținerea răspunsului virusologic susținut, dar si regresia fibrozei
hepatice. Scopul lucrării. Scopul acestui studiu a fost evaluarea fibrozei hepatice după tratamentul cu
preparate antivirale cu acțiune directă la pacienții cu ciroză hepatică cu VHC, precum și aprecierea
eficacității acestei terapii la astfel de pacienți. Material și Metode. Studiul prospectiv, randomizat, a
cuprins 74 de pacienți cu ciroză hepatică VHC, care au urmat tratament cu preparate antivirale cu acțiune
directă. Pacienții au fost repartizați în două loturi: I lot – sofosbuvir și daclatasvir/ledipasvir cu ribavirin
timp de 12 săptămâni și al II - lea lot – sofosbuvir și daclatasvir/ledipasvir timp de 24
săptămâni. Rezultate. Repartiția pacienților: I lot – 38 (51, 3%) pacienți, al II-lea lot – 36 (48,6%).
Repartizarea pacienților după stadiul de fibroză apreciat la inițierea terapiei: stadiul F3 (12, 5 – 13,9
kPa) la 4(5,4%) pacienţi, stadiul F4 – 70 (94,5%). La 6 luni și 12 luni de la finisarea terapiei s-a constatat
o amelioare a fibrozei hepatice în ambele loturi: în I lot - media valorii fibrozei hepatice la inițierea
terapiei antivirale, la 6 și 12 luni după tratament a constitut 31±14,0 kPa, 25,3±10,7kPa și respectiv
20,3±10,1kPa; în lotul II a fost 28,5±10,0kPa, 28,2±11,6kPa și respectiv 24,3±10,1 kPa. Răspuns
virusologic susținut a fost la – 68 (91, 9%) pacienți, eșec la tratament – 6 (8,1%)
pacienți. Concluzii. Schemele care conțin sofosbuvir și daclatasvir/ ledipasvir cu/fără ribavirin, au
prezentat rezultate promițătoare și sunt o opțiune bună de tratament în ciroza hepatică cu VHC.
Rezultatele studiului nostru au relevat o amelioare a fibrozei hepatice la 6 și 12 luni după această terapie.
