Association of magnesium sulfate in the standard treatment of hypertensive encephalopathy to the prehospital stage

Abstract

Background. In hypertension are deregulated intracellular homeostasis and trans-membranes transport of magnesium. Magnesium decreases not only in hypertension, but also in antihypertensive treatment, especially with diuretics and beta-blockers. Objective of the study. The importance and efficacy of magnesium sulfate administration in Hypertensive Encephalopathy to the Prehospital stage. Material and Methods. Retrospective analysis of the Prehospital Emergency Medical Service Request Sheets in Republic of Moldova from 2017 for Hypertensive and Cardiovascular Emergencies, and for the treatment of Hypertensive Crises with MgSO4 in association with standard antihypertensive treatment. Results. In Republic of Moldova, in 2017, at the prehospital stage, intravenous administration of MgSO4 in a dose from 1,0 to 5,0 g was effective in the case of patients with nonresponsive hypertensive crises on standard antihypertensive therapy, including hypertensive encephalopathy. Conclusion. MgSO4 can be an effective drug in the treatment of Hypertensive Encephalopathy and, if there are no contraindications, an intravenous dose of MgSO4 of 1,0 (1,25) – 2,5 – 5,0 g, slow, over 5 – 10 – 15 minutes, in association with standard antihypertensive treatment, is recommended.

Introducere. În hipertensiune este dereglată homeostaza intracelulară și transportul trans-membranar al magneziului. Magneziul scade nu numai în hipertensiune, dar și în tratamentul antihipertensiv, în special cu diuretice și beta-blocante. Scopul lucrării: Importanța și eficacitatea administrării magneziului sulfat în encefalopatia hipertensivă la etapa de prespital. Material și Metode. Analiza retrospectivă ale fișelor de solicitare de Asistență Medicală Urgentă Prespitalicească a Republicii Moldova, din anul 2017 cu Urgențe Cardiovasculare și Hipertensive, și tratamentul Crizelor Hipertensive cu MgSO4 în asociere cu tratamentul antihipertensiv standard. Rezultate. În Republica Moldova, în anul 2017, la etapa de prespital, administrarea intravenoasă de MgSO4 în doză de 1,0 - 5,0 g a fost eficace în cazul pacienților cu crize hipertensive non-respondente la terapia antihipertensivă standard, inclusiv encefalopatia hipertensivă. Concluzii. MgS04 poate fi un medicament eficient în tratamentul encefalopatiei hipertensive și, dacă nu sunt contraindicații, se recomandă administrarea intravenoasă a MgSO4 în doză de 1 (1, 25) – 2,5 – 5,0 g, lent, timp de 5 – 10 – 15 minute, în asociere cu tratamentul antihipertensiv standard.
