Bevacizumab for complex treatment of non-infectious uveitis

Abstract

Background. Non-infectious uveitis is the inflammation of the uvea. This group of diseases with multiple etiology are a continuous challenge for ophthalmologists. Traditional treatment of uveitis could have a lot of adverse effects, that’s why the necessity of new medicines is still actual. Objective of the study. Evaluate the therapeutic efficacy of bevacizumab for non-infectious uveitis compared with traditional method of treatment and assessment of adverse reactions. Material and Methods. This study include 9 clinical cases with non-infectious uveitis, that were treated with bevacizumab and traditional methods of treatment according to latest protocols. Follow-up period was about 1-5 years. Often, bevacizumab was administrated repeatedly (2-3 times) to stabilise the remision for 1-5 years. Results. Inflammatory process was stabilized by administration of bevacizumab in anterior chamber. The effect prolong the remission with 4 months and improve central vision. It seems that the duration of remission is significant bigger compared to traditional method of treatment and repeated administration of drug have a cumulative effect. Conclusion. Traditional method of treatment of noninfectious uveitis with steroids make the management of disease more difficult, because of the multiple adverse reactions. Bevacizumab slow down the inflammatory process and prevent the complications of uveitis, like macular oedema and neovascular glaucoma. Introducere. Uveita non-infecțioasă reprezintă inflamația tunicii vasculare. Tratamentul acestui grup de patologii este o provocare continuă pentru oftalmolgi, datorită etiologiei plurifactoriale și patofiziologiei incerte. Scopul lucrării. Aprecierea eficacității terapeutice a preparatului bevacizumab la pacienții cu uveite non-infecțioase, în comparație cu schema tradițională de tratament și studierea efectelor secundare posibile. Material și Metode. Acest studiu a cuprins 9 cazuri clinice cu diagnosticul de uveita non-infecțioasă, unde s-a utilizat ca metodă de tratament de prima linie, bevacizumab, în camera anterioară. Pacienții au fost urmăriți în dinamică timp de 1-5 ani, iar la necesitate bevacizumabul a fost administrat repetat pentru stabilizarea remisiei. Rezultate. Administrarea de bevacizumab în camera anterioara a determinat stabilizarea procesului inflamator cu instalarea remisiei de cel puțin 4 luni și îmbunătățirea vederii centrale. Efectul de durată în evoluția procesului inflamator este net superior în cazul preparatului bevacizumab spre deosebire de tratamentul tradițional, iar administrările repetate au un efect cumulativ. Concluzii. Administrarea în cantități mari a preparatelor steroidiene complică semnificativ tactica de tratament, datorită efectelor adverse importante. Preparatul bevacizumab are o acțiune benefică complexă atât asupra procesului inflamator, cât și asupra complicațiilor frecvente specifice uveitelor.
