Abstract:
Background. The ruptures of the pubic symphysis and sacroiliac joint sometimes happen during labor
with diastasis between the pelvic bones and deformation of the pelvic ring. The frequency of these
lesions is from 1:300 to 1:30,000 births, creating many problems for young women in postpartum
life. Objective of the study. To determine the criteria for early stabilization of the pelvic ring rupted
during childbirth, taking into account the individual particularities of the woman in labor. Material and
Methods. Under our follow-up there were 14 patients with disruption to the pelvic ring II-III degree
during labor. Special bed position was recommended for the patients (n=4) with rupture of the pubic
symphysis II degree. Sacroiliac joint subluxation was reduced by manual traction in multiparous patients
(n=3) with disruption of III degree. Closed pelvis osteosynthesis by external device was performed.
Results. Osteosynthesis was performed in trauma clinic taking into account that patients had breast-fed
children. The maximum anesthesia time was up to 30 minutes. Closed reposition of the pelvic bones and
stabilization by an external device without additional blood loss were performed. Reposition of damaged
bone surfaces was achieved in all cases. In 3-4 hours after surgery, the patients were returned to their
children. From the second day they were able to get up, move around the room, take care of the children.
Long-term results of treatment were evaluated from 1.5 to 14 years and rated as good. Conclusion.
Rupture of the pelvic ring over physiological norms in women in labor require qualified diagnostics and
appropriate treatment so as not to transfer young women into chronic patients. Conservative treatment
does not give an stable clinical effect in multiparous women with disruption of II degree. Introducere. În timpul nașterii uneori apare ruptura simfizei pubiene și a articulației sacroiliace cu
formarea diastazei între oasele bazinului și cu deformarea inelului pelvin. Frecvența acestor leziuni este de
la 1:300 până la 1:30.000 de nașteri, creând multe probleme femeilor în perioada post-partum. Scopul
lucrării. A determina criteriile de stabilizare precoce a inelului pelvin lezat în timpul nașterii, ținând cont
de particularitățile individuale ale lăuzelor. Material și Metode. Sub supraveghere au fost 14 paciente cu
leziuni ale bazinului de gradul II-III în timpul nașterii. Pacientelor (n=4) cu leziunea de gradul II le-a fost
recomandată poziționarea specială în pat. La multipare (n=3) cu ruperea simfizei de gradul III cu subluxația
articulației sacroiliace, ultima a fost redusă prin tracțiune manuală urmată cu osteosinteza închisă a
bazinului cu dispozitiv extern. Rezultate. Osteosinteza a fost efectuată în clinica traumatologică ținând
cont de faptul că pacientele au avut copii de alăptat. Timpul maxim de anestezie era până la 30 de minute.
S-a efectuat o repoziție închisă a oaselor pelviene și stabilizarea cu dispozitiv extern fără pierderi de sânge.
În toate cazurile s-a obținut repoziția suprafețelor osoase. La 3-4 ore după operație, pacientele s-au întors
la copii. Din a doua zi, pacientele s-au putut ridica, deplasa în cameră, avea grijă de copii. Rezultatele la
distanță ale tratamentului au fost urmărite de la 1,5 la 14 ani și au fost apreciate ca bune. Concluzii.
Diastaza între oasele pelviene la lauze mai mare decât normele fiziologice necesită diagnostic calificativ
și tratament adecvat pentru a nu transfera femeile tinere în paciente cronice. Tratamentul conservator în
cazul ruperii simfizei pubiene de gradul II la multipare nu dă efect clinic stabil.
